OBJECTIVE: To investigate efficacy of nasal cavity ventilation expansion technique alone on patients with mild and moderate obstructive sleep apnea syndrome(OSAHS).METHODS: Give 30 patients who with mild-to-moderate OSAHS to do nasal cavity structure diorthosis(different from nasal cavity expansion that put forward by the relevant scholars), and use corresponding inspection to evaluate the effectiveness of the operation 1.Acoustic rhinometry(AR), rhinomanometer(RM) were used to detect nasal cavity in 0-5cm volume(NCV0-5), nasal minimal crosssectional area(NMCA), nasal airway resistance(NAR) in 30 cases of mild and moderate OSAHS before and after operation. 2.Nasal blockage degree was evaluated with visual analogue scale(VAS). Polysomnography(PSG) was carried out in 30 cases of OSAHS before and 3-5 months after surgery. The sleep apnea and hypopnea index(AHI), lowest arterial oxygen saturation(LSa O2),Epworth sleepiness score(ESS) were used to evaluate the efficacy of the operation.3.Set up a corresponding numerical model to the mild-to-moderate patients with OSAHS,and application of numerical simulation verify the curative effect of surgery.RESULTS: 1.Preoperative NCV0-5, NMCA, NAR, VAS score, AHI, LSa O2 and ESS score were(4.83±0.08)cm3,(0.37±0.03)cm2,(0.437±0.012)k Pa·s/L, 7.31±1.29,(10.2±3.0)/h,(77.3±7.2)% and 11.2±2.6 respectively, while the parameters after operation were(8.05±0.42)cm3,(0.58±0.04)cm2,(0.202±0.014)k Pa·s/L, 2.72±0.85,(5.4±1.9)/h,(81.3±5.9)% and 7.9±1.4. There were statistical significance compared with the parameters before and after operation(P <0.01) 2.Δ VAS, Δ NAR as a result, the spearman rank correlation statistical analysis it is concluded that r = 0.065, P = 0.734,which can draw Δ VAS, Δ NAR on no statistical correlation, Δ ESS and Δ LSa O2(%) by the spearman rank correlation statistical analysis it is concluded that r = 0.157, P = 0.4 > 0.05,which can draw Δ ESS, Δ LSa O2(%) in statistical significance between two groups of data on no statistical correlation, Δ ESS, Δ AHI(times/h) by the spearman rank correlation statistical analysis it is concluded that r = 0.243, P = 0.195 > 0.05,which can draw Δ ESS, Δ AHI(times/h) no statistical correlation.3.Preoperative patients with nasal area of air pressure for- 71 pa, pharyngeal cavity flow field in the region of the pressure for- 140 pa, postoperative nasal area of the flow in the pressure for-25 pa, pharyngeal cavity area of flow field pressure- 76 pa.CONCLUSION: 1.After the objective examination: nasal sound reflection, nasal resistance, PSG, the quantitative comparison of the preoperative and postoperative proved nasal cavity structure diorthosis is effective on the treatment of mild-to-moderate OSAHS.2.Objective examination and subjective VAS score and ESS score there exists inconsistency of operation effectiveness evaluation, subjective rating remains to be further improved.3.By preoperative and postoperative nasal and Pharyngeal cavity flow field numerical simulation, it is concluded that patients with nasal and pharyngeal cavity pressure of the airflow field size back to basic with healthy people, and to make quantitative analysis of the effectiveness of the operation...
