Objective: Through the clinical results observation of using improved “T”clasp palatal retention for restoring the maxillary unilateral dentition defect,to explore the method of maxillary unilateral dentition defect with the removable partial denture.Methods: Selecting 4 patients that fitting maxillary unilateral dentition defect in the hospital which affiliated dalian medical university prosthetics from march 2013 to March 2014. males,3 、females,1, age from 54 to 68.3 patients whose unilateral remaining teeth with PFM combined crown restoration using vitallium casting stents;the other one,s remaining teeth is PFM combined crown,which using co-cr alloy casting stents.Incorporating criterion:maxillary unilateral detition defect,Abutment teeth healthy;no obvious decay、 periathritis of root and periodontal disease;Abutment teeth loose less than I degree,X ray firmed that the alveolar bone absorption doesn,t exceed 1/3 root length; Basic normal occlusal relationship.Respectively using separated “T” clasp two arms enter into undercut for retention of the maxillary remaining premolar and molar palate surface, with casting stents of the removable partial denture. After one and a half years follow-up observations, to evaluate the clinical results of the remaining abutment and restoration retention.Results: 4 patients which maxillary unilateral dentition defect with using improved “T” clasp palatal retainer were follow-up surveyed for one and a half years.3 patients that using vitalluim casting stents have no obvious discomfort, retention is good, and mucosa dense paste, chewing function is good, abutment teeth have no obvious loose, no red and swollen of the oral mucosa.1 patient that using co-cr alloy casting stent have no obvious discomfort,after six months.Feeling uncomfortable, loose restoration,chewing function decline, the food is easily to store after one year.Furthermore,the mucous membrane of edentulous was red and swollen,the remaining abutment loose is I degrees, but have no obvious periodontal pocket. It was the result of “T” clasp power reduction. Then it recovered health by using adjustment and soft relining.Conclusion:, It can better solve the retainer difficult and poor stability problems of maxillary unilateral dentition defect by using improved form of palatal “T” clasp. Using vitallium casting stents can prolong restoration retention time and improve the efficiency of chewing.
