Clinical Research On Treatment Type C Fracture Of The Distal Radius With Locking Compression Plate And External Fixator

objective: to study the clinical application of locking compression plate and external fixation stents in the treatment of type C distal radius fracture.Methods: From May 2013 to December 2014 in our hospital,there are 68 patients with type C distal radius fractures who received surgical treatment.36 patients of them are male,other 30 female,average age being 46.25 years.The locking plate treatment group(LCP)had 48 cases of patients. According to AO classification as a standard, 14 cases of distal radius fractures were C1 type, C2 type of distal radius fractures were 16 cases, 18 cases of distal radius fractures were C3 type;External fixator(EF) group had 20 cases of patients,C1 type 2, C2 type 8, C3 type 10.Postoperative review the lateral X ray film, and calculating the radius height of LCP and EF group, palm inclination Angle, and the the function recovery of wrist joint after postoperative 3 months and 1 year. The evaluation criteria was new GW(Gartland- Werley) scoring system.Results: All patients of LCP group completely healed. one patients of EF group was infected but infection was controled after treatment without appearring the serious distal radius osteomyelitis.68 patients received follow- up about 12 ~18months,average follow-up time being 14.5 months.LCP and EF group received X-ray after Postoperative. Radius height,palm inclination angle were calculated with Photoshop5 software: LCP group(9.40 士 0.52) mm, EF group in(9.20 士 0.50) mm;Palm Angle : LCP group(9.35 士 0.60) °, EF group in(9.27 士 0.71 °);tilt Angle value: LCP group(20.40 士 0.78) °, EF group(21.30 士 0.73 °).Then using statistical soft ware SPSS13.0 done t test, P > 0.05.There was no statistically significant difference for reduction and fixing between LCP and EF group operation.Three months later the LCP group mean GW score(3.51 士 1.17) points, EF group for GW score(5.36 士 1.58) points, using statistical soft ware SPSS13.0 done t test, the P < 0.05. LCP and EF group had statistically significant.Year later, LCP group GW score(2.86 士 1.01) points, EF group for GW score(3.12 士 1.00) points, using statistical soft ware SPSS13.0 done t test, P > 0.05. There was no statistically significant between LCP and EF group.Conclusions: For type C distal radius fractures, locking compression plate in the short term was in favour of the recovery of joint function.There was no difference to wrist function for long-term. Distal radius fractures with soft tissue damaging seriously use Externa l fixation,which is a good choice.
