Hypoparathyroidism And Variation Rule Of Serum Calcium After Total Thyroidectomy Of Thyroid Carcinoma

Objective：Discussion on the cause, prevention and treatment ofhypoparathyroidism (PTH) and post thyroidectomy hypocalcemia-(PHC)after total thyroidectomy of thyroid carcinoma.Methods:A retrospective analysis of Second Hospital of Jilin University from2013January to2015January in thyroid surgery for thyroid carcinomaresection in186patients, including44cases of male, female142cases,male to female ratio is about1:3.23. Record the serum parathyroidhormone and calcium levels of every patients, before1-2days of theiroperation,1,2,5days after their operation, combined with the clinicalmanifestations, to analyse the causes, prevention and treatment of thehypoparathyroidism and post thyroidectomy hypocalcemia.Results:1.After total thyroidectomy,PTH and serum calcium decreased in different degree compared with the preoperative, and the lowestnumerical of the majority of patients occurred in the se-cond days aftersurgery, and their clinical symptoms disappeared or minor after fifth days in most of them. Follow up for more than six months, found no permanenthypoparathyroidism patients.2. In186patients, hypoparathyroidism andhypocalcemia in patients about60cases, accounting for about32.26%.Higher incidence of hypoparathyroidism and hypokalemia.Conclusion:1. After total thyroidectomy of thyroid carcinoma hypoparathyroidism and hypocalcemia occurred at a higher rate, the high incidence ofoccurrence rate in central lymph node dissection and lateral neckdissection combined with only thyroidectomy patients.2. The key to prevent the occurrence of hypothyroidism and postthyroidctomy hypocalcemia is the protection of parathyroid surgery, especially the protection of parathyroid gland in situ.3.The occurrence of hypothyroidism and hypocalcemia after totalthyroidectomy in patients with calcium supplement should follow theprinciple of individuation.
