PURPOSEThis paper aims to investigate the causes and features of the failure of hiparthroplasty through analysis of failed cases of hip arthroplasty revision surgery.METHODSA retrospective analysis of237failure cases of hip arthroplasty after Orthopedicrevision surgery row has been made in Sino-Japanese Friendship Hospital of JilinUniversity from September2009to December2014:It included163failure cases (68.8%)in other hospital and74failure cases (31.2%) in Sino-Japanese Friendship Hospital;128male cases (54.0%),109female cases (46.0%); The average age of the initial permutationis53.4years(19years~86years), the average age at revision surgery is59.1years (21years~87years). It covered127cases of osteonecrosis (53.6%),78cases of fractures (32.9%),16cases of developmental hip dysplasia (6.8%),7rheumatoid arthritis cases (3.0%),6cases of tonic spondylitis (2.5%),2cases of tuberculosis of the hip (0.8%),1cases of bonetumor (0.4%) from diagnosis of primary disease. It involved130cases of uncemented(54.9%),77cases of bone cement (32.5%),17cases of reverse hybrid (cementedacetabular cup, uncemented femoral stem)(7.2%),13cases of mixed (uncementedacetabular cup, cemented femoral stem)(5.5%) in prosthesis types of the initial hiparthroplasty.RESULTSFrom failure factors of the initial hip arthroplasty,it included118cases of Asepticloosening (50.2%),73cases of infection (30.8%),19cases of dislocation (7.6%),9casesof lining wear (3.8%),9cases of periprosthetic fractures (3.4%),9cases of others(iliopsoas collision, femoral fracture, femoral head prosthesis, prosthetic poor position tofriction, periprosthetic ossification, false limbs extended)(3.8%).The average of prosthesissurvival age is5.7(from the primary arthroplasty to the revision years)(0.0to29.0), the reverse hybrid is9.3years, cemented is7.4years, hybrid is5.1years, uncemented is4.3years. The average age the prosthesis survival in all kinds of reasons are following:liningwear is16.5years, aseptic loosening is7.3years, the other is7.0years, periprostheticfracture is4.9years, infection is2.9years, and dislocation0.2years is.It included136cases of Early failure of postoperative in the initial arthroplasty (<5years)(57.4%),54cases of medium-term failure(5-10years)(22.8%) and47cases of late failure(>10years)(19.8%).CONCLUSIONThe average age of failure cases of prosthesis survival after hip arthroplasty is5.7years. The main causes for failure included aseptic loosening (50.2%), infection (30.8%),dislocation (8.0%), lining wear (3.8%), periprosthetic fractures (3.4%), others (3.8%).
