Objective:Diabetes mellitus is a metabolic disorder,caused by absolutely or relativelyabsence of insulin.The main characteristics of that maintain a high biood glucose level.The number of diabetes is increasing tremendously in the recent years with theimprovement of living standards.Bring heavy burden to society and economy.Themost common complications of type2diabetes mellitus is macroangiopathy.Butalso the important reason of the death, disability. Is a serious threat to the survival ofdiabetes patients and quality of life.homocysteine is methionine metabolism processof a kind of reactive vascular injury of amino acids,Many studies have shown thatboth at home and abroad,between homocysteine and atherosclerosis have a closerelation.Some epidemiological studies and experimental observations have provedthat homocysteine was a new independent risk factor of vascular diseases.Hcy canaffect the incidence of diabetic vascular complications.To explore the change of Hcyin different stage of type2diabetes mellitus as well as its macrovasularcomplications.To research the relationship between the Hcy and type2diabetesmellitus as well as its macrovascular complications.Method:55patients of T2DM were choosed as a subject of the study who are in theendocrine department from The Third clinical medical school of The Jilin Universityfrom September2012to November2014,Male:30cases, female:25cases,and whohave be diagnosed as diabetic according to the publication of Diabetes diagnosis andclassification standard of WHO in1999. Among them,30cases were withmacrovascular complications(MVC group),Male:17cases, female:13cases,age:from25to80.Other25cases without macrovascular complications(Non-MVC group),Male:13cases, female:12cases,age:from20to85. In addition,normal controls(NC group)of25cases were set to facilitate comparison at the same time,who choosed from our hospital physical examinationers,male:14cases, female:11cases, age from40to70.To get the general clinical features of all and to measure the height andweight.FPG、2hPG、HbAlc、CRP、lipids(TC,TG、HDL—C、LDL—C)、Hcy andso on were detected and BMl were calculated.All the statistical analysises were deaJtwith SPSS17.0software。And then to analysis the relationships of the indicators.Results:1.Compared with the nomal control group,the level of gender、age in type2diabetic patients (Non—MVC group and MVC group) have no differencesSignificant(P>0.01).the level of BMI increased significantly, the differences werestatistically significant(P<0.01).The course of disease in MVC group is longer thanNon—MVC group,the differences were statistically significant(P<0.01).2.The level of FPG、FINS、HOMA-IR、TC、TG、LDL-C in NC group、Non—MVC group and MVC group are increase gradually,the differences werestatistically significant(P<0.01)。 the level of HDL-C is decrease gradually.thedifferences were statistically significant(P<0.01).Compare with nomal controlgroup,the level of2hPG、HbAlc in Non—MVC group and MVC group are increasedsignificantly(P<0.01),Folic Acid is decrease gradually(P<0.01).But betweenNon—MVC group and MVC group,have no differences Significant(P>0.05).3.The level of Hcy in nomal control group and type2diabetic patients areincreased significantly,Compared three groups,the differences were statisticallysignificant(P<0.01).the level of CRP in the three groups are increasegradually(P<0.01).4. Pearson correlation analysis suggested that the level of Hcy wereproportional to Course of disease、BMI、FPG、2hPG、HbAlc、FINS、HOMA-IR、CRP分别(r=0.402,r=0.808, r=0.883,r=0.863, r=0.730,r=0.918;r=0.916,r=0.969,P<0.01).the level of Hcy were negative correlation to Folic Acid(r=-0.937,P<0.01).the level of Hcy were not proportional to age、TC、TG、HDL—C andLDL—C.Conclusion:1.Hcy is involved in macroangiopathy in typr2diabetes.It is meaningful forearly diagnosis of type2diabetes. 2. Hcy is one of the indicators for effective monitoing as the risk factor in type2diabetes.
