Purpose:To study early diabetic retinopathy and the change rule of visual contrast sensitivity,and significancein of visual functionin in early DR;To observe the change of DR visual contrast sensitivity and the relationship of TCM symptoms of DR.Methods:According to the prospective clinical research method, Included in the diabetic retinopathy patients79cases that Conform to the criteria for the DR1to DR4, the subjects to general situation, TCM syndrome, the related biochemical indexes and the routine inspection of ophthalmology, using the CSV-1000visual function test system, the examination of contrast sensitivity in different spatial frequency, DR international clinical standard grouped by2002and analysis.Results:1. Different grade of DR, in both the bright and dark conditions, the CS spectrum value had statistical significance (p<0.01).Bright condition, in the3c/d,12c/d spectrum,DR2,DR3,DR4CS are DR1CS have significant differences (p<0.01);In the6c/d,18c/d spectrum,DR3, DR4CS are DR1, DR2CS have significant differences (p<0.01);Under scotopic condition, in the3c/d spectrum, DR2,DR3, DR4CS are DR1CS have significant differences (p<0.01);in the6c/d,12c/d,18c/d spectrum, DR3, DR4CS are DR1, DR2CS have significant differences (p<0.01).2. In both the bright and dark conditions, DR at all levels of CS drop difference spectrum, high frequency band (12c/d,18c/d) CS decreased more significantly (p<0.01).Bright condition, the level DR1,12c/d spectrum of CS changed to3c/d spectrum of CS change had significant difference (p<0.01),18c/d spectrum of CS change to3c/d,6c/d,12c/d spectrum of CS change have significant difference (p<0.01);the level DR2,12c/d spectrum of CS change to3c/d,6c/d spectrum of CS change have significant difference (p<0.01),18c/d spectrum of CS change to3c/d,6c/d,12c/d spectrum of CS change have significant difference (p<0.01);the level DR3,12c/d spectrum of CS changed to3c/d spectrum of CS change have significant difference (p<0.01),18c/d spectrum of CS change to3c/d,6c/d spectrum of CS change have significant difference (p<0.01);the level DR4,12c/d spectrum of CS change to3c/d,6c/d spectrum of CS change have significant difference (p<0.01),18c/d spectrum of CS change to3c/d,6c/d,12c/d spectrum of CS change have significant difference (p<0.01).Scotopic condition, the level DR1,12c/d spectrum of CS changed to3c/d spectrum of CS have significant difference (p<0.01),18c/d spectrum of CS change to3c/d,6c/d spectrum of CS change have significant difference (p<0.01);the level DR2,12c/d,18c/d spectrum of CS change to3c/d,6c/d spectrum of CS change have significant difference (p<0.01); the level DR3,6c/d spectrum of CS change to3c/d spectrum of CS have significant difference (p<0.01),12c/d,18c/d spectrum of CS change to3c/d,6c/d spectrum of CS change have significant difference (p<0.01);the level DR4,12c/d spectrum of CS change to3c/d,6c/d spectrum of CS change have significant difference (p<0.01),18c/d spectrum of CS change to3c/d,6c/d,12c/d spectrum of CS change have significant difference (p<0.01).3.7kinds of common TCM syndrome types, DR patients in the state of bright,6c/d spectrum of CS change has significant correlation with Yang deficiency syndrome, blood stasis (p<0.05).18c/d spectrum of CS change has significant correlation with qi deficiency syndromes (p<0.05).In the scotopia,3c/d spectrum of CS change has significant correlation with qi deficiency syndrome, blood stasis (p<0.05).6c/d spectrum of CS change has significant correlation with qi deficiency syndrome and Yang deficiency syndrome (p<0.05).12c/d spectrum of CS change has significant correlation with Yang deficiency syndrome (p<0.05).18c/d spectrum of CS change has significant correlation with Yang deficiency syndrome (p<0.05).4. The bright condition, spectrum (6c/d) CS value have significant correlation with the urine trace albumin (p<0.05).Scotopic condition, low frequency band (3c/d) of the CS value have significant correlation with the urine trace albumin (p<0.05), the spectrum (6c/d) of the CS value have significant correlation with urine trace albumin and fasting plasma glucose (p<0.05);High frequencies (12c/d,18c/d) of the CS value have significant correlation with duration of fasting blood glucose,(p<0.05). 5. DR patients at all levels of glycated hemoglobin,triglycerid,β2microglobulin, urine trace albumin group had statistically significant difference between (p<0.05);DR at all levels of patients’blood urea nitrogen, serum creatinine, uric acid, total cholesterol and fasting glucose group had no statistical significance difference between (p>0.05).6. DM course of rank correlation analysis, DR course and grading is strongly associated with DR classification (p<0.01), and positively correlated, namely, the longer the duration of DM, the heavier DR condition.Conclusion:1. Each CS decline of early DR patients obviousy, as DR illness aggravating, full spectrum CS threshold overall showed a trend of gradual decline, and at high frequencies (12c/d,18c/d) CS decline is more apparent.Patients with early DR, visual function, has appeared different degree of damage, the retinal ganglion cells damage evident of macular area, and as the illness is aggravating, the more serious the visual function damage. CS is a sensitive indicator of assessment of visual function in patients with early DR.2.7kinds of common TCM syndrome type in DR patients, no matter in bright and dark conditions, qi deficiency syndrome, blood stasis, Yang deficiency syndrome has significant correlation with CS.CS find objective reference index and the basis for TCM syndrome classification and outcome of DR, treats his bluff, better to improve the patient’s quality of life for patients.3. The clinical course of patients with early DR, the change of CS has affected with fasting blood sugar, urine trace albumin directly, and negatively correlated with them.
