Complete Mesocolon Resection (CME) In Right Colon Surgery

Objective:This retrospective study was designed to compare the traditional surgeryoperation in colon cancer patients with complete mesocolon resection of colon cancercases，to explore mesocolon complete excision (CME) preliminary application in coloncancer surgery operation.Methods: Collect30cases of traditional colon cancer surgery and30cases of CME（theage, comorbidities, treatment group and other factors of patients are controlled）of***Medical College Yijishan Hospital from2009-2011。A retrospective comparativeanalysis of two groups of patients was done to explore mesocolon complete excision(CME) preliminary application in colon cancer surgery operation，by comparing theaspect of surgical feasibility, safety, short and long term effects, etc.Results: There was no significant difference between the general information oftraditional surgery group and the CME group, so they were comparable. The blood lossin operation of Conventional surgery group was (354.7±234.1) ml, complete resectionmesocolon group was (230.9±194.6) ml,so the blood lose of CME group wassignificantly less than traditional surgery group. The number of cases of intraoperativeblood transfusion in Conventional surgery group was8cases, blood transfusion rate was 26.6%, the number of cases of CME Group was5, blood transfusion rate of16.6%, sothe case of traditional surgery group was significantly more than the traditional CMEGroup. The postoperative bowel resection length of traditional radical surgery groupwas (7.3±2.1) cm, complete mesocolon resection group was (18.3±1.7) cm, so thelength of traditional group was shorter than the length of CME Group. The number oflymph nodes of the traditional group was (13.4±5.1), the CME group was (23.3±6.7).There was significant difference in the cases of total incidence of postoperativecomplications between conventional surgery group and CME group, but the case ofpostoperative chylous leakage of conventional surgery group was1, CME Group was3,so the case of conventional surgery group significantly less than CME Group. Thesurvival rate after1year of Traditional group is lower than the CME group, and thelocal recurrence rate is higher than the CME group; The survival rate after3year ofTraditional group is lower than the CME group, and the local recurrence rate is higherthan the CME group.Conclusion: Complete mesocolon resection (CME) is a safe, feasible operation methodin colon cancer, the short-term effect is good, is expected to become standardized righthalf colon cancer surgery operation.
