Chronic diseases such as hypertension and diabetes seriously threaten to human health. The World Health Organization(WHO) have explicitly pointed out that the chronic diseases can be prevented and controlled and that the most effective approach to control them is to control the risk factors which induce these diseases based on community. At present the construction of community health care in our country is not perfect. Community-based care and medical service which always have a cross and repetition with the hospital service system need more improvement. There are many difficulties and obstacles in the work of our health management system. It will be greatly significant if our community health service can provide effective and individual health service for those who have high risk of developing chronic diseases.At present the main strategy of controlling chronic diseases in our country is a control mode based on epidemic survey and pointed to the crowd as its object. Based on the study of the crowd we hope to establish a state control method based on person’s physiological state and pointed to an individual person which can help community doctors to give intense lifestyle interventions (mainly diet and physical activity) to individual. In this paper313(12out) chronic diseases high-risk people from a community of Zhejiang province and19(1out) volunteers (more than35years old) from Hangzhou accepted interventions based on binary control or personal state control. The community people were divided to low-risk group and high-risk group according to the assessment of risk factors of chronic diseases. They accepted different levels of intervention according to the binary control strategy. One year later96%of the high-risk group (22persons) turned to be healthy. At the same time we tracked changes on the long-term health of the19volunteers. With their data we explored a personal state control strategy pointed to individual and using the method of artificial neural networks we make a preliminary attempt to build a mathematical model between physiological status of individual (blood pressure, blood glucose, etc) and their risk factors of chronic diseases. The reliability of the models is tested by residual analysis. The results shows that coincidence rates of measured parameters of the18models are all more than80percent (p>0.05). So we can think that the modeling approach about personal risk factors of chronic diseases based on neural network is feasible, and it provide the feasibility of further study of personal state control method.
