ObjectiveTo investigate the risk factors, clinical characteristics and relationship between antibiotic therapy and prognosis of carbapenem-resistant Acinetobacter baumannii infection.MethodsThirty cases whose CRAB isolation and culture were positive and who were confirmed with nosocomial infection in2nd affiliated hospital medical school of Zhejiang University between July2006and July2008were retrospectively investigated.ResultsThe average age of the patients was65.6years old and the average length of staying in hospital was60.8days. The hospital acquired pneumonia accounted for the first, accounting for56.7%.There were29patients who had been in intensive care unit(ICU)within2weeks and22were conscious disturbance,29received urethral catheterization,27inserted with central venous catheter,25received tracheal intubation and23assisted with mechanical ventilation. There were22patients who were co-infected with other bacteria and7were co-infected with fungi.Before CRAB-positive culture,11(36.7%)experienced>4kinds of antibiotics with the majority of β-lactamase inhibitors and carbapenems.In thirty cas-es of CRAB,9cases of sensitive to cefoperazone-sulbactam. The clinical therapeutic effectiveness in15cases,whose treatment is cefoperazone-sulbactam.The mortality was50%in patients with CRAB infection,including10cases of cefoperazone-sulbactam.ConclusionsCRAB infection frequently occurrs in patients who prolonged stay in hospital, exposured to carbapenems and other antibiotics.Invasive procedures including arteriovenous intubation, trachea cannula and mechanical ventilation could increase the infection opportunities. Cefoperazone-sulbactam has a certain effect in the treatment of CRAB.
