ObjectiveResearch on the the change and prognosis of the indexes of platelet activation andoxidative stress in acute exacerbations of chronic obstructive cardiopulmonary disease(AECOPD) patients.MethodsEighty cases of chronic obstructive pulmonary disease patients (forty AECOPD withoutpulmonary hypertension, and forty AECOPD with pulmonary hypertension, thirty threewith respiratory failure and forty seven without respiratory failure) and thirty healthycontrols were included. Platelet activation factor (PAF),11-dehydro-thromboxane B2(11-DH-TXB2), P-Selectin (CD62P), Superoxide Dismutase (SOD), Glutathione peroxidase(GSH-Px) levels were detected by ELISA method.Results1. In the AECOPD group, PAF,11-DH-TXB2and CD62P levels were significantly higherthan healthy control group, but SOD and GSH-Px levels were significantly lower thanhealthy controls (P<0.05).2. In the AECOPD with pulmonary hypertension (30mmHg or higher) group, PAF,11-DH-TXB2, and CD62P levels obviously higher than that of AECOPD patients withoutpulmonary hypertension(pulmonary artery pressure<30mmHg), but SOD and GSH-Pxwere significantly lower (P<0.05).3. In the AECOPD patients with respiratory failure (artery oxygen pressure <60mmHg),PAF,11-DH-TXB2and CD62P level were obviously higher, but SOD and GSH-Px weresignificantly lower than those without respiratory failure (artery oxygen pressure≥60mmHg)(P<0.05). 4. In the AECOPD patients with moderate to severe pulmonary hypertension (41mmHg orhigher), PAF,11-DH-TXB2and CD62P levels were obviously higher than that of patientswith mild pulmonary hypertension (less than40mmHg), but SOD and GSH-Px levelswere significantly lower than the mild patients (P<0.05).5. In the patients suffered two or more times from COPD per year, PAF,11-DH-TXB2andCD62P levels were higher than those suffered less than two times, but SOD and GSH-Pxlevels were lower (P<0.05).Conclusions1. PAF,11-DH-TXB2and CD62P levels in AECOPD patients were significantly elevated,which show the state of platelet activation, and the patients complicated pulmonaryhypertension and hypoxemia were especially noticeable, which were related to the level ofpulmonary artery pressure.2. SOD and GSH-Px levels in AECOPD patients were significantly depressed, which showthe state of oxidative stress, and the patients complicated pulmonary hypertension andhypoxemia were especially noticeable, and the oxidative stress in patients with moderate tosevere pulmonary hypertension were prominent compared to mild pulmonaryhypertension.3. The state of platelet activation and oxidative stress were related to frequency ofAECOPD, which indicate more risk and poorer prognosis.4. The state of platelet activation and oxidative stress were related to repeatedexacerbations and continued progress of COPD and complicated hypoxemia andpulmonary hypertension.
