Objective:To assess efficacy and safety of tonifying kidney and inducing ovulation decoctionthrought observing its clinical effect on infertility caused by polycystic ovary syndrome(PCOS), and to provide TCM with new thoughts and methods to treat infertility caused byPCOS.Methods:We collected out-patients of department of reproductive medicine with infertilitycaused by PCOS from December2012to June2013. Fifty patients were selected accordingto inclusion and exclusion criteria of both TCM and modern medicine, and were dividedinto observation group and control group randomly,25patients of each group.Theobservation group was treated with tonifying kidney and inducing ovulation decoction,while control groups with clomiphene. Metformin was added in two groups duringtherapeutic process as supplementary treatment.Same indicators was chose to monitorclinical effect, changes of symptoms, ovulation and pregnancy rate, changes of sexhormone level, changes of insulin resistance, changes of blood fat and adverse reactionsbefore and after treatment. All the data were statistically analyzed to evaluate clinicalefficacy of tonifying kidney and inducing ovulation decoction on infertility caused byPCOS, and summarize and analyze clinical understanding of tonifying kidney and inducingovulation decoction in treating this disease.Results:1.clinical effect:Observation group:8recovery cases, occupied32%,6significantly effective cases,occupied24%,8effective cases, occupied32%,3ineffective cases, occupied12%.Twenty-two cases are effective in total, effective rate was88%. Control group:4recoverycases, occupied16%,3significantly effective cases, occupied12%,3effective cases,occupied12%,15ineffective cases, occupied60%. Ten cases are effective in total,effective rate was40%. The two groups were significantly different (P<0.05), clinical effects of observation group was much better than that of control group.Effectiveness of TCM syndromes:Observation group:8recovery cases, occupied32%,6significantly effective cases,occupied24%,8effective cases, occupied32%,3ineffective cases, occupied12%.Twenty-two cases are effective in total, effective rate was88%. Control group:3recoverycases, occupied12%,4significantly effective cases, occupied16%,3effective cases,occupied12%,15ineffective cases, occupied60%. Ten cases are effective in total,effective rate was40%. The two groups were significantly different (P<0.05),effectiveness of TCM syndromes of observation group was much better than that of controlgroup.2.Scores of TCM symptoms:BMI, hirsutism score, acne score, score of TCM symptoms were all descended aftertreatment in both two groups, the difference was significant (P<0.05), and efficacy ofobservation group was better than that of control group.3.Ovulation and pregnancy:When compared ovulation and pregnancy condition of two groups, the observationgroup was equal to control group (P>0.05). Twenty-one cases had dominant follicle,effective rate was84%; Nineteen cases had dominant follicle, effective rate was76%.Nineteen ovulation cases in observation group, ovulation rate was76%; Eighteenovulation cases in observation group, ovulation rate was72%. Eight pregnancy cases inobservation group, pregnancy rate was32%;4pregnancy cases in observation group,pregnancy rate was16%.4.Hormone level:LH, LH/FSH, T and ER2Rwere all descended after treatment in two groups, and FSHwas ascended, the difference was significant statistically (P<0.05). Levels of FSH,LH/FSH and T were much better in observation group. LH and ER2Rwere equal in twogroups (P>0.05).5.Insulin resistance:FBG, FINS and HOMA-IR were all descended after treatment in two groups, the difference was significant statistically (P<0.05). Levels of FBG was much better inobservation group (P<0.05). FINSã€HOMA-IR were equal in two groups (P>0.05).6. Blood fat:TG, HDL, LDL were all descended after treatment in two groups, the difference wassignificant statistically (P<0.05). Levels of TG, HDL, LDL were equal in two groups(P>0.05).7. Adverse drug reaction (ADR):One ADR in observation group, occupied4%; five ADRs in control group, occupied20%. It indicated that tonifying kidney and inducing ovulation decoction had fewer ADRConclusion:Tonifying kidney and inducing ovulation decoction was effective in treating infertilitycaused by PCOS, and was equal to clomiphene in improving basic temperature, increasingdominant follicle, inducing ovulation, decreasing insulin resistance and blood fat.Tonifyingkidney and inducing ovulation decoction was more effective than clomiphene in improvingTCM syndromes, adjust endocrine, increasing pregnancy rate and reducing ADRs.Especially in improving TCM syndrome and reducing ADRs, tonifying kidney andinducing ovulation decoction had notable superiority. |
