Early Enteral Nutrition Combined With Probiotics Effect On The Nutritional Status Of The Patients With Hypertensive Intracerebral Hemorrhage

Objective: Patients with hypertension intracerebralhemorrhage of the body often severe metabolic disorders, autonomicnerve regulation function disorder and nerve conduction abnormalities,showed high decomposition, high metabolism; at the same time, theimmune function was seriously inhibited, easily lead to the seriousinfection, even multiple organ failure. Thus affect the normalgastrointestinal function, it is difficult to guarantee the effective enteralnutrition, the body is the negative nitrogen balance, further suppressionof immune function, resulting in mortality, morbidity increasedsignificantly. Therefore, to restore normal gastrointestinal function inpatients with hypertensive intracerebral hemorrhage, correct imbalancesof nutritional status of the body and improve the immune function topromote the recovery of neurological function in patients is particularlyimportant. Combined with probiotics enteral nutrition in the use ofcolorectal malignancy during peri operation period, which caneffectively prevent the imbalance of intestinal flora, promote intestinaland liver function recovery, improve the nutritional status, enhanceimmunity, reduce postoperative infection rate. But at present theprobiotics used in study of severe hypertensive intracerebral hemorrhage patients less, it is difficult to determine the probiotics for the treatmentof severe hypertensive intracerebral hemorrhage patients can play abeneficial role. To investigate the effect of adding probiotics onnutritional status, immune function in patients with hypertensiveintracerebral hemorrhage and prognosis of early enteral nutrition,provide a reference basis for the clinical treatment of hypertensiveintracerebral hemorrhage. Methods: During February2013to January2014in Zigong No.4People’s Hospital affiliated to Luzhou MedicalCollege, the50cases of patients with hypertensive intracerebralhemorrhage were randomly divided into the treatment group and thecontrol group, there were no significant differences between the twogroups of patients age, sex ratio and body general aspects (p>0.05). Twogroups of patients after admission in48~72h started to undergo earlyenteral nutrition, enteral nutrition in the treatment group based on theaddition of probiotics. On the1th,7th,14th and21th days after enteralnutrition beginning, such targets as serum total protein, albumin,hemoglobin and lymphocyte count, upper arm circumference, upper armmuscles of two head of skinfold thickness were observed duringhospitalization respectively. Patients have gastrointestinal bleeding,intestinal flora imbalance and pulmonary infection were observed, andthe recent recovery of consciousness were observed too. Result:1.Effect of probiotics on nutritional status indicators: Serum total protein, albumin, hemoglobin and lymphocyte count in patients of the twogroups at each time point between the differences were not statisticallysignificant (p>0.05).On the seventh day after nutritional support ofprobiotics the index of the treatment group and the control group had nostatistical difference (p>0.05). On the fourteenth day, the levels of totalserum protein, albumin, hemoglobin, lymphocyte count in the treatmentgroup was higher than the control group, but no significant difference(p>0.05). On the twenty-first day, the levels of total serum protein,albumin, hemoglobin, lymphocyte count in the probiotics treatmentgroup were higher than the control group, with statistical difference(p<0.05). There is no statistical difference in the changes of upper armcircumference and arm muscles of two head of skinfold thicknessbetween the probiotics treatment group received nutritional supporttreatment of the seventh days and the control group(p>0.05). On thefourteenth day and the twenty-first day, the decline degree of upper armcircumference and arm muscles of two head of skinfold thickness in theprobiotics treatment group were lower than the control group, withstatistical difference (p<0.05).2. Effect of probiotics on gastrointestinalbleeding: Within21days of nutritional support, probiotics therapygroup development of digestive tract hemorrhage in the number of caseswas less than the control group, there was statistically significantdifference (p<0.05), and the incidence of gastrointestinal hemorrhage in patients with the treatment time of probiotic treatment group was shorterthan the control group (p<0.05).3. Effect of probiotics on intestinal flora:Within21days of nutritional support, probiotics treatment groupoccurred intestinal flora were less than those in control group, there wasstatistically significant difference (p<0.05), correcting intestinal floraimbalance with time on probiotics in the treatment group was shorterthan the control group (p<0.05).4. Effect probiotics on the incidence ofpulmonary infection: Within21days of nutritional support, probiotictreatment difference compared with the control group was no differenceon pulmonary infection rate (p>0.05). But antibiotic use in the probiotictreatment group was shorter than the control group, with statisticaldifference (p<0.05).5. The recent recovery of consciousness: Within21days of nutritional support, the patients with consciousness in treatmentof probiotics treatment group were more than that in control group, withstatistical difference (p<0.05). On the60th days, the patients withconsciousness in treatment of treatment group were more than that incontrol group, but no statistical difference (p>0.05). Conclusion: Earlyenteral nutrition can improve the nutritional status of adding probioticsand bowel dysfunction in hypertensive intracerebral hemorrhage, reducethe incidence of patients with digestive tract hemorrhage, improve thenutritional status of patients in order to enhance the body’s immunity, soas to improve the ability of anti infection in patients with pulmonary infection, shorten the treatment time. The patients have a certain role inpromoting awareness of the turn for the better, is conducive to therecovery of neural function in patients with late stage.
