objective:This thesis aims to compare and analyze the different therapeutic effect of DHS and PFNAapplied in the treatment of the intertrochanteric fractures (EvansII and EvansIII type) among elderlypatients and will provide a theoretical basis for selecting the clinical method of the above-mentioned twotypes of senile intertrochanteric fractures.Method:A retrospective analysis on the clinical cases of DHS and PFNA treatment on both Evans II andEvans III type senile intertrochanteric fractures has been done, with cases from the First Affiliated Hospitalof Shihezi University from December of2007to December of2011. Ultimately37cases have beenselected in terms of the unified diagnostic criteria, inclusive criteria and exclusive criteria, including24cases of dynamic hip screw (DHS group) and13cases of anti-rotation type of proximal femoral nail (PFNAgroup). Analysis on homogeneity of variance is done on the aspect of age, gender, etc.. Obtain data fromX-ray evaluation of fracture healing, checking patients’ ipsilateral hip function one year after surgeryaccording to Harris hip scoring criteria, recording the postoperative complications, duration of hospital stay,hospital costs, surgery time and blood loss situation by means of the postoperative follow-up and finallymake a statistical analysis.Result:1.The two surgical options for treating senile intertrochanteric fractures turn out to be notstatistically significant (P>0.05) in terms of age and gender difference, with two sets of data equated witheach other and comparable.2.Comparison between DHS and PFNA in terms of Harris hip joint functionscoring, fitness rate after surgery, postoperative complications, hospital stay, duration of operation andblood loss shows that there is no statistical significance in their difference(P>0.05). While comparison onhospitalization expenses shows statistical significance (P<0.05) and comparison on fracture healing time isalso statistically significant(P<0.05).Conclusion:Compared DHS with PFNA in case of treating senile Evans II and Evans III type ofintertrochanteric fractures, the former costs lower hospitalization expenses but takes longer fracture healingtime.
