Qingkuiyuyangtang With Traditional Chinese Medicine Enema In Treatment Of Ulcerative Colitis Mechanism Research

Objective: To observe Qingkuiyuyangtang with enema for ulcerative colitis clinicalefficacy, as well as anorectal dynamics of platelet activation, cytokines, and to explore itsmechanism of action in ulcerative colitis.Methods: September2008-May2010, Jinan Military General Hospital, Departmentof Gastroenterology, patients in72cases, meet the diagnostic criteria for ulcerative colitisand Western medicine damp card type standards. Randomly divided into two groups,namely the experimental group and control group of36cases. Experimental group weregiven Qingkuiyuyangtang and medicine retention enema in the control group was givenenteric-coated tablet of mesalazine. The total treatment lasted3months. Symptom scorebefore and after treatment, overall efficacy of TCM treatment, improvement in colonicmucosal histology and detection of anorectal dynamics and measured platelet α-granulemembrane glycoprotein (CD62p), lysosomal membrane glycoprotein (CD63),thromboxane A2(TXA2) and platelet adhesion rate, maximum aggregation rate, testinginterleukin-18(IL-18), tumor necrosis factor-α (TNF-α), soluble interleukin-2receptor(sIL-2R), interleukin-10(IL-10) and other cytokines.Results: The total effective rate was94.44%in the control group, the total effectiverate was77.78%, the test group than the control group (P <0.05). Symptoms wereimproved, traditional Chinese medicine syndromes, colonic mucosal lesions andhistological improvement in the experimental group were significantly better than thecontrol group (P <0.05). Detection in the anorectal dynamics, the experimental groupsignificantly increased rectal sensation threshold, defecation threshold and pain thresholdand rectal compliance, was superior to the control group (P <0.05); in platelet activation,the test group can significantly reduce the CD62P, CD63, TXA2, platelet adhesion rate and the maximum aggregation rate was superior to the control group (P <0.05); cytokines, thetreatment group significantly reduced IL-18, TNF-α, sIL-2R expression, increased levelsof IL-10expression, was superior to the control group (P <0.05).Conclusion: The combination of Qingkuiyuyangtang ulcerative colitis enema is aneffective treatment method, can significantly improve the performance of patients withsymptoms and colonic mucosa. The mechanism may be by improving the rectal dynamics,inhibiting platelet activation, regulating levels of cytokines play a role.
