Objective:To discuss and analyze the related factors of the postoperative hydrocephalus caused byaneurysmal subarachnoid hemorrhage.Materials and Methods:The medical records of106patients with aneurysmal subarachnoid hemorrhage treated inthe first department of Neurosurgery of the Second Bethune hospital of Jilin University fromJuly2013to January2015were retrospectively studied.According to our standard,80cases ofthe106patients satisfied the inclusion criteria.On the basis of the resaults of the patients’postoperative and discharged computed tomography,80cases were divided into2groupsincluding17hydrocephalic patients and63non-hydrocephalic patients.According topublished literature and the patients’ specific treatment,we chose gender,age,location ofaneurysmal,Hunt-Hess classification,Fisher classification,the postoperative amount ofsubarachnoid hemorrhage,hypertension,hyponatremia,times of bleeding,hematoma volume ofventricle, surgical options,lumbar cisterna drainge,diabetes,operation opportunity,intracranialinfection as possible related factors to compare,while the non-hydrocephalic patients werematched group.Multi-factor regression analysis was used to analyse these statisticalsignificance factors,picked out and further analyse the factors related of the postoperativehydrocephalus caused by aneurysmal subarachnoid hemorrhage.Results:The resaults of statistic,compare and analysis of the above15possible related factorsshowed that gender,location of aneurysmal,surgical options,diabetes,hypertension andoperation opportunity were non-statistical significance factors while age,Hunt-Hessclassification,Fisher classification,the postoperative amount of subarachnoid hemorrhage,hyponatremia,times of bleeding,hematoma volume of ventricle,surgical options,intracranialinfection were statistical significance factors and lumbar cisterna drainge was a protectivefactor. The decreasing array of the facors according to logistic regression analysis washematoma volume of ventricle,the postoperative amount of subarachnoid hemorrhage,hyponatremia,Hunt-Hess classification,Fisher classification,age, intracranial infection andtimes of bleeding. Conclusion:Many factors interacted with each other resaulting in the occur of the postoperativehydrocephalus caused by aneurysmal subarachnoid hemorrhage.The decreasing array of thefacors were hematoma volume of ventricle,the postoperative amount of subarachnoidhemorrhage,hyponatremia,Hunt-Hess classification,Fisher classification,age, intracranialinfection and times of bleeding. These patients who were large hematoma volume ofventricle,high Hunt-Hess classification should be eliminated hematoma clearly andpostoperative continuous lumbar cisterna drainge was necessary. The above methods wasvery useful to avoid the hydrocephalus caused by aneurysmal subarachnoid hemorrhage.
