Objective: To explore the effects of semi-hip arthroplasty and totalhip arthroplasty in treatment of intertrochanteric fracture in the elderlypatients of type2diabetes.Methods: From March2011to March2014,37patients ofintertrochanteric fracture and type2diabetes had been retrospectivelyanalysised in the China-Japan Union Hospital of Jilin University. Thepatients were divided into two groups: semi-hip arthroplasty group (HA,n=22) and total hip arthroplasty group (THA, n=15). Age, sex, causes ofinjury, medical combinations, hospitalization time, operation time,bleeding volume during operation, postoperative drainage volume,postoperative ambulation starting time, postoperative complications wererecorded for both groups of patients. The Harris scores were analyzed atthe time of discharge as well as three years after operation, so as toevaluate the therapeutic effect of two kinds of surgical operation.Results:(1)37elderly patients of intertrochanteric fracture and type2diabetes had been collected from our hospital. There were22semi-hiparthroplasty22patients and15total hip arthroplasty patients, of whom10patients were male and27were female,with ages ranging from70to90,and the average age was77.16±6.26.(2) Comparison of general information: Compared with THA group,the ages were higher in HA group (P<0.05). There was no significantdifference in sex, causes of injury and medical combinations.(3) Comparison of evaluation index: Compared with THA group, the bleeding volume during operation was less in HA group(270.45±126.92VS.726.67±412.69), the postoperative drainage volume was also less inHA group (165.91±54.31VS.233.33±72.38). The starting time ofpostoperative ambulation was shorter (7.18±1.65VS.8.80±1.82). Therewas no significant difference in hospitalization time, operation time andpostoperative complications.(4) Harris scores: the Harris score of THA group was higher than HAgroup both during the time the patients were discharged and three yearslater.Conclusion:Semi-hip arthroplasty in treatment of intertrochanteric fracture hadsignificant short-term clinical effect in elderly patients of type2Diabetes,but total hip arthroplasty had obvious advantage in long-term effect.
