Objective: To investigate the characteristics and effects of cervicalspinal cord injury of without fracture and dislocation with anterior-column soft tissue injury.Methods: From December2012to July2014,seventy-one patientswith cervical spinal cord injury of without fracture and dislocation treatedin our hospital were retrospectively reviewed.All patient underwentX-ray and MRI.We tool transversal high or intermediate signal intensityon sagittal T2-weighted image of anterior longitudinal ligament andabnormal high signal intensity of disc on sagittal T2-weighted and asstandard of acute anterior-column soft tissue injury,and eiher can bereceived indication of surgical exploration. ALL patients underwentanterior surgery or posterior-anterior surgery, we probed suspicioussegments and these segments were treated by stabilitysurgery.Anterior-column soft tissue injury were surely found byoperation,and were recorded Injury Group(42cases),no injury patientsrecorded None Injury Group(29cases).We compared Injury Groupwith None Injury Group on ages,injuried factors,cervical curvaturedecrease,prevertebral shadow broadening,JOA scores and JOA scoresimprovement rate.Results: Forty-two cases of Injury Group,twenty-nine cases of NoneInjury Group.ALL patients underwent mean18.1months follow-up(range from3months to25months).Mean age of Injury Group is48.2,and meanage of None-Injury Group is53.1.The proportion of young,middle-aged,the old is16.67%,76.19%,7.14%in Injury Group,6.9%,62.07%,31.03%in None Injury Group.The proportion of traffic accidents,fall damage,falling accidents,crunshing by weight and others is17%,38%,19%,21%,5%in Injury Group,is41%,31%,4%,24%,0%in None Injury Group.The proportion of cervical curvature decreases is85.71%(36cases)inInjury Group,and65.52%(19cases)in None Injury Group.The differencehas statistic significance(P<0.05).The proportion of prevertebral shadowbroadening is95.24%(40cases)in Injury Group,and75.86%(22cases) inNone Injury Group.The difference has statistic significance(P<0.05).JOA scores in Injury Group improved from5.02±4.06to11.50±4.32,and JOA improvement rate is62.35%±17.34%.JOA scores inNone Injury Scores improved from5.02±4.06to12.67±4.61,and JOAimprovemet rate is65.34%±17.34%.In both groups postoperative JOAscores are higher than preoperatve JOA score,and the difference hasstatistic significance(P<0.05).It indicated that neurological functiongreatly improved by surgery. The preoperative,posteroperative JOAscores and JOA scores improvement rate compared between twogroups,and the difference has statistic significance(P<0.05).It showedthat the damage of neurological function in anterior-soft tissue injurywere more severe than none injury one,and recovery of neurological function in anterior-soft tissue injury is worse than none injury one.Conclusion: Onest age of CSCIWFD with anterior-column softtissue injury is younger than simple CSCIWFD,and cervical curvaturedecreases may be preprocess factor;all damage,falling accidents,crunshing by weight are main damage factors,and prevertebral shadow isalways becoming broaden after injury;Nerological damage of CSCIWFDwith anterior-column soft tissue injury is much serious,but Patientsreceived surgury as soon as possible,neurological function still achieved acertain degree recovery.
