Objective: To analyze the curative effect of anterior surgery of lower cervical spine with posterior structural injury and explore its indications.Methods: Twenty-eight cases of patients with cervical spine fracture and dislocation treated in our hospital from January 2015 to November 2017 had vertebral fractures and dislocations,intervertebral disc injury and neurological symptoms.Fractures and dislocations of intervertebral joints were separately or simultaneously found.Vertebral plate fracture,spinous process fracture,posterior ligament complex injury instability after the back of the symptoms.Twenty-eight patients underwent different surgical procedures and 10 patients underwent anterior surgery.Cervical vertebrae flexion was performed after the anterior approach was performed and the stability of the cervical vertebra was evaluated.The experimental group was selected as the experimental group.18 patients with anterior and posterior combined surgery,as the control group.Follow-up of the improvement of neurological function,imaging assessment of cervical stability,etcThe two groups of patients age,hospital stay,operation time,intraoperative blood loss,postoperative drainage,intervertebral height,Cobb angle,JOAAnd VAS score.Results: There was no significant difference in age and hospital stay between experimental group and control group(P> 0.05).The experimental group had longer operation time,more intraoperative blood loss,more postoperative drainage,the difference was statistically significant(P <0.05).The postoperative intervertebral height,Cobb angle,JOA and VAS score of two groups were all better than preoperative,the difference was statistically significant(P<0.05);while the preoperative and postoperative intervertebral height,Cobb angle,JOA,VAS score had no significant difference(P<0.05).In addition to 1 patient did not improve,all patients Frankel grading improved1-2 level,cervical stability maintained well,the implant is in good condition.Conclusion: The lower cervical spine fractures with posterior structural injuries are evaluated by anterior flexion radiographs after anterior surgery.The stable cases can be treated with posterior approach to obtain satisfactory curative effect with the advantages of small injury.
