BACKGROUND: Cervical spinal cord injury without fracture or dislocation can be divided into different types according to causative factors. Most of scholars insist operation is the preference treatment on cervical spinal cord injury without fracture or dislocation. Different surgical operations were performed according to different types. There are there surgical approcchs: anterior, posterior and anterior-posterior operation.OBJECTIVE: To explore the clinical effect of early anterior-posterior operation for cervical spinal cord injury without fracture or dislocation.METHODS: we reviewed 58 cases of cervical spinal cord injury without fracture or dislocation from June,2003 to March,2008.12 cases of these patients suffered from the decrease of the storing space of spinal canal combined with cervical disk herniation. There were 9 males and 3 females with an average age of 60.2 years. X-ray film and CT scans revealed neither fracture nor subluxation,ossification of posterior longitudinal ligament 4cases,hypertrophic ligamentum flavum 2 cases,developmental spinal stenosis 6 cases. MRI showed cervical disk herniation in all cases. 12 cases all received early anterior-posterior operation.6 cases received operation in 24 hours;4cases in 48 hours;2 cases in 72 hours. The myeloid function were evaluated by ASIA grade. RESULTS: All patients were followed up form 6 to 30 months,the averaged period was 16 months. All segments with anterior fixation attained solid fusion without loosening or breakage implants. No re-closed was expanded with spinal cord decompression. The myeloid function recoverd at different extents in all cases except for one that had no improvement. ASIA grade: A grade 1 cases, B grade 8 cases ,C grade 3 cases before operation. A grade 1 cases, C grade 5 cases, D grade 5 cases ,E grade 1 cases,after operation.CONCLUSIONS: early anterior-Posterior operation is a effective method to cure cervical spinal cord injury without fracture or dislocation.
