Purpose:This study aims to compare the differences between the mutiple clinico-pathologic factors of synchronous and metachronous bilateral primary breast cancer, bilateral primary breast cancer(BPBC) and unilateral breast cancer(UBC).Methods:The study is based on149women with histological verified breast cancer diagnosed between1998.6and2013.6at Department of Surgical Oncology of Sir Run Run Shaw Hospital, including34patients with BPBC. All women’s charts were reviewed. All recurrences and deaths were registered. Factors like age, menstrual/procreating/breastfeeding factor, operation method, radiation history, tumor size, lymph node metastasis state, TNM stage, pathology diagosis, molecular subtypes and prognosis are compared.Results:For the first cancer, mBPBC group is younger for than sBPBC group(45.0±7.0vs57.1±8.6), p<0.001; more patients of mBPBC are under40years than patients of sBPBC(36.8%vs0), p=0.011; more NO state of lymph node metastasis (86.6%vs40.0%) and less N1-3state in mBPBC than in sBPBC,(p=0.041); less PR positive state in mBPBC than in sBPBC(42.9%vs86.7%), p=0.021. For the second cancer, less not-infiltrating or early stage cancer in mBPBC than in sBPBC(15.8%vs53.3%), p=0.030. For the first cancer, tumors of the size of Tis-T1state in BPBC are more than UBC(60.7%vs37.4%), p=0.029. The differences in survival between the BPBC and UBC has no significance. Conclusions:Age is a most significant risk factor for BPBC, as mBPBC is more to happen in young women. The age of the first cancer of mBPBC group is5-10years earlier than sBPBC and UBC groups. mBPBC is more likely to have higher grade lymph node metastasis state, less PR positive expression than sBPBC in the first cancer; for the second cancer, mBPBC is less likely to have not-infiltrating or early-stage cancer. There’s no differences in the clinico-pathologic characteristics between the first cancer and the second cancer. BPBC is more likely to have bigger size of tumor than UBC. There is no significant correlation of Overall Survival.
