Effect Of Metformin For Non-obesity Women With Polycystic Ovary Syndrome Undergoing IVF-ET Treatment

AIM:To explore the effect of metformin in non-obese women with PCOS undergoing in vitro fertilization (IVF).METHODS:Retrospective data analysis of non-obese patients with PCOS undergoing IVF/ICSI treatment using a long GnRH agonist protocol in women’s hospital school of Medicine Zhejiang University from2005to2013.In half of the patients, patients(n=240) received metformin(1000to1500mg) daily, starting the cycle prior to gonadotropin treatment. The main outcome measures were doses of Gn, duration of stimulation, the serum estradiol level on the day of HCG treatment, total number of oocytes, mature oocytes, fertilization, embryos, the rate of OHSS, clinical pregnancy rates.RESULTS:In patients treated with metformin, there was no difference in the total dose of FSH required per cycle (1965±480U vs2130±510U) and duration for stimulation(12.1±3.3d vs13.2±3.4d).the total number of follicles on the day of hCG treatment was decreased (13±4.2VS16±7.2) with no change in follicles>14mm in diameter (10±3.6VS11±3.4); Metformin treatment did not affect the mean number of oocytes retrieved (11.9±3.8vs12.3±2.7). However, Fertilization rates (60%vs39%)、 effective embryo rate (70.5±20.8%vs66.2±20.3%), clinical pregnancy (43.3%VS21%) are also increased. Furthermore, a significant decrease in the incidence of cancellation rate of transferred embryo (9.1%vs18.3%) and severe ovarian hyperstimulation syndrome (OHSS)(3.8%vs10.4%)was observed.CONCLUSION:Short-term co-treatment with metformin for patients with PCOS undergoing IVF/ICSI cycles does not improve the response to stimulation but significantly improves the pregnancy outcome and reduces the risk of OHSS.
