ObjectivesTo assess the current situation of insulin injection technique through questionaire survey, and analyze the related factors affecting insulin injection.According to the results of this survey,we implement patients individual intensified insulin injection technology education,observe the effect of this education,evaluate the influence on the mastery of insulin injection technique and glycemic control,in order to find more effective methods to deal with and solve the problem of glycemic control brought by the lack of injection knowledge and error injection technology.MethodsThis research adopts the methods of questionnaire and cognitive approach.lt proceeded in three stages:The first stage,in order to understand the current situation of insulin injection technique in patients with diabetes,we surveyed221patients from department of endocrinology in out-patient and ward in a hospital in Zhejiang province who treated with insulin;The second stage,according to80patients willing to participate in the group accepted individual intensified education,we measured the baseline levels of HhA1c,FBG,PBG,then face to face teaching method,follow-up intensive education every month;The third stage,followed-up six months,a second questionaires were surveyed and another HbA1c,FBG,PBG were measured.Data was analyzed by SPSS17.0software package.Results1The current situation of insulin injection technology in patients with diabetes is not optimistic.Overall,87.33%of patients use shorter needles.There is no relationship between length of needle and insulin leakage,lipohypertrophy,HbAlc (P>0.05). The insulin leakage on skin surface and needle tip dose not affect the control of HbAlc(P>0.05).It has significantly incidence increased on insulin leakage when a skin-pinch maintain time less than5seconds(P<0.05). After injection,the time the needle under the skin is related with the leakage of needle tip,and has statistically significant(P<0.05).There was a significant relationship between needle reuse,rotate injection at a same site and lipohypertrophy(r=-0.292,P<0.05).2After individual intensified education,the HbA1c,FBG,PBG were improved,the difference was statistically significant(P<0.05);There were13projects involved injection technology,which patients mastered were better than before education, the difference was statistically significant(P<0.05).Conclusion1.The insulin injection skill of patients with diabetes are poor, and the issues of needle reuse, lipohypertrophy and incorrect rotation of injection sites were severe. Needle reuse and incorrect rotation of injection sites may relate to the incidence of lipohypertrophy. A single needle should be used one time and correct rotation of injection sites should be emphasized in education of insulin injection technique.2.Individual intensified education in insulin injection technique leads to an improvement not only in diabetic patient’s technology of insulin injection but also in glycemic control in insulin-treated diabetic patients.
