| Objective:1.To comprehend the occurring rate of lipohypertrophy(LH) on the insulin injected position in the diabetic patients in Chongqing as well as its related factors.2.To investigate the rate and risk factors of LH in different grades hospitals.And to provide theoretical basis for improving instruction methods of insulin injection.3.To survey the monthly expense of patients with LH. And provide the data basis for optimizing the distribution of medical resources and saving the cost of economic hygiene.Methods:Patients who had injected insulin for more than one year from 10 hospitals including 4 tertiary hospitals, 3 secondary hospitals, and 3 primary hospitals were observed by questionnaire, inspection and palpation of injection site. The occurring rate of LH on the insulin injected position was evaluated.Results:There were 540 patients had LH among 736 diabetic patients and the rate of LH was 73.4%. Among these LH patients, there were 58.01% in the tertiary ospitals,87.08% in the secondary hospitals, and 82.33% in the primary hospitals. The LH was related to duration of diabetes, expense of needle, injection site rotation, injection interval(P<0.05). The incidence of LH in the injection sites was associated with the duration of insulin injection and the injection distance in the tertiary hospitals; the duration of insulin injection and the injection distance in the secondary hospitals; and the diabetes duration and the injection distance in the primary hospitals(P<0.05).The glycosylated hemoglobin(Hb A1c) and disease treatment cost(monthly medical cost) among patients with LH was higher than those without. The differences were statistically significant(P<0.05).Conclusion: The LH is a common problem in insulin-treated patients, and it is mainly caused by not standardized insulin injection behavior. The economic burden of patients, families and society also will increase because of LH. Medical staffs need to strengthen the concept of standardization of insulin injection and to promote standardization injection. So that the incident of LH and the economic burden can be reduced, the glycemic control can be improved. |
