Objective:Describe the epidemiologic features of inpatients in the orthopaedic hospital inShenyang. The distribution of the injured area, type, and cause of patients withfracture because of accidental injury are analyzed. And the distribution of the injuryreason, hospitalization days and the hospitalization expense are analyzed. Based onthis, we can promote the measures of the diagnosis and accident prevention.Method:By collecting the gender, age, residence, injury causes, injury area and injurytype, the time of first diagnosis, hospitalization days and total hospitalization expenses,we recorded on the data by Epidata, and analyze the statistic by SPSS19.0. Thedistribution of constituent ratio is used to analyze the classification indicators, and thechi-square test is used to analyze the comparison among groups; The median(interquartile range) is used to describe the measurement data, and the Kruskal-WallisTest is used to describe the interblock.Result:(1) There were5,061inpatients meet the inclusion criteria, with male64.63%(3271/5061) and female35.37%(1790/5061), and the male female ratio was1.8:1. The age of patients suffered from accidental injury ranges from1to98yearsold. Patients aged between40and59years old had a higher incidence of accidentalinjury. The ratio of young adults in the male is higher than the female. The ratio of theold patients in the female is higher than the male. For the place of accidental injury,there were3,365(66.49%) urban patients and1,696(33.51%) rural patients, the ratiobetween urban and rural was1.98:1. Analyzed the time when accidental injuryhappened, we found that it was more common in the periods between7:00AM and9:00AM and between7:00PM and8:00PM. For the month of the injury happened,there was a higher incidence of accidental injury in the period between May andOctober.(2) For the part in body of the injury, the constituent ratio of fracture in hand orfoot was the highest (18.53%), following the fracture of thigh-bone (16.87%), fractureof tibia and fibula(11.97%), fracture of radius and ulna(9.33%). The constitution of multiple parts of the fracture is minimum(0.24%). For the type of accidental injury,91.98%of the patients had injuries in upper or lower limb, and then the spine is6.71%, the rib is0.79%, the pelvis is0.28%, the multiple fracture is0.24%.(3) The reason of accidental injury is accident (45.43%), break (28.10%), crush(10.97%), sport(5.69%), falling down(5.24%), incision(3.30%), sprain(1.28%). Thereason of accidental injury showed significant difference between different agegroups(χ2=1450.4,P<0.001), The reason of accidental injury showed significantdifference between different sex group(sχ2=204.8,P<0.001), The reason of accidentalinjury showed significant difference between urban and rural(χ2=47.903,P<0.001).(4) The maximum of inpatient days is179days and the minimum is1day. Theaverage value is16.2±15.7days. The inpatient days showed significant differencebetween different reason of accidental injury(H=128.381,P<0.001).The reason ofinpatient days showed significant difference different type of accidental injury.(H=349.930,P<0.001).(5) The maximum of hospitalization expense is179309.0and the minimumis320.0. The average value is23161.2±20264.0.The median is17291.0andthe inter-quartile range is23031.5. The hospitalization expense showed significantdifference between different reason of accidental injury(H=173.111,P<0.001).Thereason of hospitalization expense showed significant difference different type ofaccidental injury(H=600.958,P<0.001).Conclusion:(1) Young and middle-aged inpatients with fracture were more common in thishospital, and the proportion is63.88%.And the proportion between male inpatientsand female inpatients is1.8:1.The occurrence of the accidental injuries was seasonalwith two incidence peaks in morning and evening. Inpatients from urban areas weremore than their counterparts from rural areas.(2) The most common reason of accidental injury was traffic accidence,following falling hurt, and the two proportions are45.43%and28.10%. Children andyoung inpatients had higher incidence in traffic accidence, and elderly inpatients hadhigher incidence in falling injuries. The most common type of accidental injury isupper and lower extremity, accounting for91.98%. Hand and foot and femur fracturesaccounted for the highest proportion, with35.40%percent. (3) The number of hospitalized days and the hospitalization expense weredifferent among inpatients with different types of injury and different reasons.Inpatients with frequent injuries and with injury in spinal column had longerhospitalization time and higher hospitalization expense. Inpatients with injury causedby traffic accident and twisting had longer hospitalization time and higherhospitalization expense.
