Every year, about500,000-1,000,000hepatitis B patients are reported in ourcountry and almost300,000cases may die of liver cirrhosis or liver cancer. HepatitisB itself has become one of three major infectious diseases in our country. Because ofvarious factors affected, it has been known that chronic hepatitis B patients could bemore prone to suffering from psychological problems, such as severe fear,disappointment, pessimism, anxiety and depression etc., in the development of thedisease. These problems may decrease their quality of life and obstacle the recoveryof disease. In the clinical context, most of the health workers were still lack of theawareness of these mental problems right now and usually use symptomatic treatmentonly so that many patients might not be effectively treated and have poor quality oflife, which could aggravate the psychological burden of patients with hepatitis B. Inpresent study, We are aim to propose some targeted preventive strategies by analyzingthe factors affecting the mental health of hepatitis B patients so as to improve thepublic awareness of mental health that hepatitis B patients suffered from, and providebasic evidence for targeted intervention as well. The results of our research could beuseful in helping patients escape from bad emotion, face the disease with positiveattitudes and take effective treatment as well. Besides, the outcome of our researchcould play a leading role for the prevention, treatment and prognosis of chronichepatitis B.Objectives: To analyze the mental health status of hepatitis B patients and theinfluential factors so that health workers may have much more understanding ofchronic hepatitis B patients with bad psychological status, and urge health workers notonly to pay attention to the disease itself, but also guidance and intervention of mentalhealth. With the intervention attached in this paper, patients could drop someeconomic pressures and then advance quality of life. These findings could be useful inproviding valuable evidence for the prevention, treatment and prognosis of hepatitisB. Methods: Participants in the study were recruited and diagnosed in the outpatientdepartment of infectious disease hospital in Sipin from Oct.2013to Sep.2014. Thequestionnaires were used for the investigation which included basic clinicalinformation, CHQ-12, Self-Rating Anxiety Scale (SAS), Self-rating depression scale(SDS) and some other relevant information. Data was typed by the program ofEPIDATA. We also performed Chi-Square test to compare the different rates betweentwo groups by SPSS16.0and Logistic regression was applied to analyze the factorsthat have impact on the general health status and mental health.Results:(1) Among patients involved in present study, the positive rate was49.2%by usingCHQ-12. The rate was48.2%in males,51.1%in females, the difference betweendifferent sexes was not statistical significant (χ2=0.295,P=0.587); The rate was49.1%,45.4%and58.0%in <30,30-49,>50age groups, respectively and thedifference among these groups was also out of statistical significance (χ2=4.411,P=0.110). We found that education, income, working status, family history of liverdisease, years of illness and treatment of anti-virus were associated with CHQ-12+.Higher income, joblessness were protective factor to mental health of patients withchronic hepatitis B (OR<1). Patients with longer years of illness and anti-virustreatment had less risks of mental problems than those without (OR>1).(2) For384cases with chronic hepatitis B, the positive rate of anxiety tendencies was41.41%. The difference between sexes were not significant (χ2=1.081,P=0.298), so asage (χ2=1.292,P=0.524). Patients with higher income and vaccination of hepatitis hadless risks of anxiety tendency than those without (OR<1). While Patients with longeryears of illness and anti-virus treatment had higher risks (OR>1).(3) The rate of depressive tendencies was42.25%among384patients. The differencewas not significant between sexes (χ2=0.092,P=0.762) and ages (χ2=3.045,P=0.218).Patients with higher income had less risks of depression than those with lower income(OR<1). Whereas patients with longer years of illness and anti-virus treatment hadhigher risks (OR>1)Conclusion(1) Chronic hepatitis B patients usually suffered from bad mental health status. The rate of general negative mental health, anxiety tendencies and depressive tendenciesin patients were all higher than in controls.(2) The risk factors might be longer years of illness and anti-virus treatment for themental health of chronic hepatitis B patients, while higher income, joblessness andvaccination of hepatitis B could be protective factors.(3) We should pay more attention to general mental health, anxiety and depressivecondition for patients with chronic hepatitis B and it is better to have psychologicalcounseling and psychological intervention for these people so that patients could bemore positive and optimistic when they are in face of the disease and then improve theeffectiveness of treatment and quality of life.
