Abstract/Summary:
Objectives:This study was conducted to assess the effect of the theory of midnight-midday ebb-flow to guide rhubarb powder acupoint sticking therapy on patients with constipation combining polyethylene glycol (PEG) solution on bowel cleansing before colonoscopy and to establish a nursing method with traditional Chinese medicine (TCM) characteristics on bowel cleansing. This method is aim to improve the quality of bowel cleansing, relief discomforts during the bowel preparation, improve patient compliance and lay the foundation of a safe, effective and feasible plan of bowel preparation before colonoscopy. Methods:A total of 66patients, during October 2014 to March 2015, were randomized by random number table method into two groups (control group [n=33]; observation group [n=33])All of 66 patients received regular treatments of gastroenterology department and symptomatic treatments according to conditions of patients. Before colonoscopy, patients were informed of relative precautions and instructed to have semi-liquid diet(SLD) three days before the examination, have white rice porridge as dinner at the day before the examination and have absolute diet at breakfast and lunch at the day of colonoscopy. Observed the last discharge of patients, if it was clear liquid stools and then performed the examination in the afternoon. Observation group:combined rhubarb powder with ethyl alcohol to make every acupoint sticking in 1cm×1cm size and 0.5cm thickness; at 6:00 to 6:30 in the morning of colonoscopy, cleaned the acupiont with normal saline and pasted the acupoint sticking on the Shenque acupoint for 4-6 hours; instructed patients to consume 2000mL of PEG solution in several times at 7:00 to 8:00 in that morning; took off the acupoint sticking before the start of colonoscopy or at the last discharge of clear liquid stools. Control group:only instructed patients to consume 2000mL of PEG solution in several times at 7:00 to 8:00 in the morning. To observe cleaning effects of the two groups, tolerance degree of bowel preparation, the first timing of exhaust and defecation and required time to achieve clear liquid stools. Results:All of 66 patients completed this clinical observation. During this observation, one patient had sever distension and didn’t defecate, after cleansing enema symptoms relieved; one patient had nausea, without special handling symptoms relieved.1. Sex, age, height, weight, job specification and syndrome differentiation of TCM were no significantly different for the two groups (P>0.05), therefore the two groups had comparability;2. The degree of bowel cleansing and tolerance degree of bowel preparation in the two groups were significantly different (P=0.002<0.01, P=0.009<0.01);3. The first timing of exhaust and defecation of the two groups was no significantly different (P>0.05), but required timing of bowel preparation of the two groups was significantly different (P=0.010<0.05).Conclusions:1. Rhubarb powder acupoint sticking therapy guided by the theory of the Midnight-midday ebb-flow on patients with constipation combining PEG solution on bowel cleansing before colonoscopy had remarkable results of improving effect of bowel cleansing and tolerance degree of bowel preparation and reducing required time of bowel preparation;2. Rhubarb powder acupoint sticking therapy was easy to conduct, and this therapy was useful to cleansing bowel for patients with constipation but without severe contraindication;3. The results of rhubarb powder acupoint sticking therapy guided by the theory of the Midnight-midday ebb-flow on patients with constipation combining PEG solution on bowel cleansing before colonoscopy fully reflect the advantage of the theory of the Midnight-midday ebb-flow on intervention time in traditional Chinese medicine nursing.
