Study On The Correlation Between TCM Syndromes With Lung Imaging And Gene Mutation For Multidrug-resistant Tuberculosis

Purpose:To explore the general distribution characteristic, correlation between TCM syndromes with extent of lung disease and hole situations, correlation between TCM syndromes with multi-drug resistant gene mutation for multi-drug resistant tuberculosis, initially identify the TCM syndromes distribution rules and objective indications for multi-drug resistant tuberculosis.Method:Used clinical epidemiology methods, to studies of correlation between distribution of TCM syndromes with imaging of 120 patients with multi-drug resistant tuberculosis. It included design and statistical classification tables, labeling syndromes, gender, age, course of disease, lung disease, the range of hole, and the resistance gene mutation sites of entry. To establishment of Excel database, distribution of TCM syndromes, determine the number, frequency, and statistical comparison. Used the modern detecting technology of gene chip, the determination of Mycobacterium of rifampicin and isoniazid resistance gene mutation, and the incidence between the various types of syndromes were analyzed and compared.Result:120 cases of multi-drug resistant tuberculosis patients with TCM Syndrome Distribution sort followed by Yinxuhuowang syndrome (40.83%), Qiyinliangxu syndrome (38.33%), Yinyangliangxu syndrome (11.67%), Feiyinkuixu syndrome (9.17%). Compared with the Yinxuhuowang syndrome and Qiyinliangxu syndrome, the range of lung lesions of Feiyinkuixu syndrome was smaller, but the difference among there groups were not significant statistical significance. Compared with Feiyinkuixu syndrome, Qiyinliangxu syndrome and Yinyangliangxu syndrome, the numbers of holes in lung were increased, but the difference among there groups were not significant statistical significance. The gene mutation of Rifampicin multi drug resistant tuberculosis mainly focused on rpoB-531 (62.50%) and rpoB-526 (19.17%). On the rpoB-531 gene mutation, compared with Feiyinkuixu syndrome and Qiyinliangxu syndrome, the occurrence rate of Yinxuhuowang syndrome and Qiyinliangxu syndrome was increased significantly. On the rpoB-526 gene mutation, compared with Yinxuhuowang syndrome, Qiyinliangxu syndrome and Yinyangliangxu syndrome, the occurrence rate of Feiyinkuixu syndrome was increased significantly. The gene mutation of Isoniazid multi drug resistant tuberculosis was on KatG315. Compared with Yinxuhuowang syndrome, Qiyinliangxu syndrome and Yinyangliangxu syndrome, the occurrence rate of Feiyinkuixu syndrome was increased significantly. On the inhAl 5 gene mutation, compared with Feiyinkuixu syndrome, Yinxuhuowang syndrome and Qiyinliangxu syndrome, the occurrence rate of Yinyangliangxu syndrome was increased significantly.Conclusion; MDR-TB syndromes distribution order of Yinxuhuowang syndrome, Qiyinliangxu syndrome, Yinyangliangxu syndrome and Feiyinkuixu syndrome. Compared with the Yinxuhuowang syndrome and Qiyinliangxu syndrome, the range of lung lesions of Feiyinkuixu syndrome was smaller. Compared with Feiyinkuixu syndrome, Qiyinliangxu syndrome and Yinyangliangxu syndrome, the numbers of holes in lung were increased. MDR-TB resistant to rifampin loci on rpoB-526 and/or KatG315 mutations as indications for Feiyinkuixu syndrome, inhA15 mutation as indications for Yinyangliangxu syndrome, rpoB-531 mutation as indications for Yinxuhuowang syndrome and Qiyinliangxu syndrome.
