Objective:Through the collection and analysis of clinical features and biochemical indices data got from 86 women who were first diagnosed as phlegm wet type polycystic ovarian syndrome (PCOS) and 27 healthy women, the present study was to explore the differences between the experiential group,women diagnosed as phlegm wet type PCOS with or without insulin resistance (IR), and the control group,which leads to enlightment of treatment methods of the diseases and prevention methods of long-term complications.Methods:A total of 86 women met the inclusion criteria were recruited at the First Affiliated Hospital of Heilongjiang University of Chinese Medicine Department of gynecology clinic and 27 control women were recruited among medical center from November 2013 to November 2014. According to HOMA-IR≥2.69, patients with phlegm wet type PCOS were divided into two subgroups. Subjects with HOMA-IR≥2.69were put into phlegm wet type PCOS+IR subgroup and other subjects were put into phlegm wet type PCOS+NIR subgroup. All the subjects’age, weight and other clinical indicators are recorded at the same time, and their sex hormones 6 items, OGTT, IRT, serum lipids were also measured and recorded. All the data was analyzed using SPSS21.0 statistical software package.Results:(1)The age and BMI of phlegm wet type PCOS+IR subgroup, phlegm wet type PCOS+NIR subgroup and the control group show no statistically significant difference(P>0.05). However the BMI mean of the three subgroups subjects are all larger than 25, which means those subjects can be diagnosed as obese.The WHR mean is also all larger than 0.8, which indicates abdominal obesity. The difference among age of menarche, hirsutism score, the incidence of a skin are significant between the control group,and the phlegm wet type PCOS+IR subgroup as well as the phlegm wet type PCOS+NIR subgroup (p<0.05).(2)Phlegm wet type PCOS+IR subgroup, phlegm wet type PCOS+NIR subgroup hadhigher LH, LH/FSH, E2, T, AND and FAI (p<0.05). Phlegm wet type PCOS+IR subgroup had lower SHBG levels, compared with phlegm wet type PCOS+NIR subgroup and the control group (p<0.05).(3)Compared with the control group, phlegm wet type PCOS+IR subgroup had higher TC, TG, LDL and lower HDL, APOA(p<0.05). Compared with the control group, phlegm wet type PCOS+NIR subgroup had higher TC, HDL, LDL and APOA(p<0.01). Compared with the phlegm wet type PCOS+NIR subgroup, phlegm wet type PCOS+IR subgroup had higher TG and lower HDL(p<0.05).(4)Compared with the control group, phlegm wet type PCOS+IR subgroup had higher glucose and insulin levels, GAUB, IAUB(p<0.05). Compared with the phlegm wet type PCOS+NIR subgroup, phlegm wet type PCOS+IR subgroup had higher FPG,2h plasma glucose and insulin levels, GAUB, IAUB(p<0.05). (5)BMI, WHR, FAI, FPG,2h plasma glucose and APOB/APOA are positively correlated with HOMA-IR index. LH, SHBG and HDL are negatively correlated with HOMA-IR index.Conclusions:(1)IR aggravated glucose and lipid metabolism on patients with phlegm wet type PCOS.(2)SHBG is negatively correlated with HOMA-IR index, serum SHBG concentration decreases when phlegm wet type PCOS with IR, which increasing incidence of hyperandrogenism.
