Objective:To Study expression of transforming growth factor beta(TGF-β) and Smad2/3 protein in the outer membrane chronic of subdural hematoma(CSDH), to explore the relationship between the abnormal expression and the pathogenesis of chronic subdural hematoma. Methods:As 18 cases of elderly patients with chronic subdural hematoma confirmed by skull CT or MRI, We collected dura mater and hematoma outer membrane in the process of drilling drainage clinical surgery by strictly aseptic operation without enzyme;While dura mater could be divided into control group, the hematoma outer membrane was divided into the experimental group, detect and analyze TGF-β and Smad2/3 protein levels in specimens respectively by using the SP 0f immunohistochemical and western blotting method. Results: 1. TGF-β, Smad2/3 was mainly expressed in capillary endothelial cells and fibroblasts cytoplasm and nuclei,the positive rate of TGF-β in the experimental group and control group were 88.9%(16/18) and 11.1%(2/18);the positive rate of Smad2/3 in the experimental group and control group were 77.8%(14/18) and 16.7%(3/18), the expression of TGF-β and Smad2/3 in the experimental group are significantly higher than the control group, with significant difference(P < 0.05), the difference was statisticallysignificant. 2.The expression amount of TGF-β was detected in the experimental group and control group were 0.317±0.028 μg/ml and 0.100 ± 0.020μ g/ml, the expression amount of Smad2/3 was detected in the experimental group and control group were 0.408±0.028 μg/ml, 0.135± 0.024 μg/ml.the expression amount of TGF-β and Smad2/3 in the experimental group was significantly higher than the control group, with significant difference(P < 0.05), the difference was statistically significant. 3.The Pearson correlation coefficient r of the expression of TGF-β and Smad2/3 in the experimental group is 0.519(P = 0.027 < 0.05). 4.The expression of TGF-β and Smad2/3 in the experimental group and the patient’s age, gender, presence of trauma history had no significant correlation(P > 0.05). Conclusions:1.A large number of new capillaries exist in the outer membrane of hematoma, part of the vascular basilemma incomplete.2.TGF-β and Smad2/3 highly expressed in the outer membrane of hematoma,may play an important role in the pathogenesis of CSDH.3.The expressions of TGF-β and Smad2/3 were not significant correlation with some clinical characteristics,such as age, gender, history of injury or not.4.We can inhibit the generation of TGF-β and Smad2/3,inhibit the process of fibrosis,promote the mature newborn blood capillary,the treatment maybe become the effective means in CSDH non-surgical treatment and prevention of postoperative recurrence.
