Objective:To investigate disinfection status of private clinics in Gusu district of Suzhou, put forward the countermeasures, and to secure environment for medical treatment.Methods:Accordance with the "People’s Republic of China Disease Prevention Act" and the relevant provisions of the "disinfection management approach", 87 and 81 private clinics in 2013 and 2014 in Gusu district of Suzhou were surveyed, respectively. Types of clinics included dental clinics, internal medicine clinics, clinics carrying out invasive treatments such as acupuncture and Chinese medicine, other clinics not carrying out invasive treatments. The test items consisted of hand hygiene of medical staff, clinics countertops(total numbers of colonies), sterilizing medical equipments(Salmonella, Staphylococcus aureus, and total numbers of colonies) as well as ultraviolet light irradiance. The number of samples was 485 in 2013 and 523 in 2014 respectively. Sampling and testing methods were based on the 2012 version of "hospital disinfectant health standards."Results:I. Investigation on disinfection status of private clinics in Gusu district of Suzhou City in 20131. Different test items results: the detection of total 117 samples of medical staff hand hygiene indicated that 99 samples were qualified, the qualified rate was 84.62%; countertops were detected in 168 clinics, 155 was qualified and the qualified rate was 92.26%; sterilization of medical devices were detected in 87 clinics, 86 was qualified and the qualified rate was 98.85%; 113 samples were detected for UV lamp and 94 was qualified and the qualified rate was.83.19%.2. Different types of clinics results: the detection of total 257 samples in 37 dental clinics showed that 230 was qualified, the average pass rate was 89.49%; among 135 samples in 29 internal medicine clinics, 121 was qualified, the average pass rate was 89.63%; among 60 samples in 12 clinics carrying out acupuncture or other invasive treatments, 57 was qualified, the average pass rate was 95.00%; the detection of 33 samples in 9 clinics not carrying out invasive treatments indicated that 26 was qualified and the average pass rate was only 78.79%.II. Investigation on disinfection status of private clinics in Gusu district of Suzhou City in 20141. Different test items results: the detection of total 119 samples of medical staff hand hygiene indicated that 98 samples were qualified, the qualified rate was 82.35%; countertops were detected in 127 clinics, 126 was qualified and the qualified rate was 99.21%; sterilization of medical devices were detected in 99 clinics, 98 was qualified and the qualified rate was 98.99%; 124 samples were detected for UV lamp and 112 was qualified and the qualified rate was 90.32%.2. Different types of clinics results: the detection of total 284 samples in 38 dental clinics showed that 267 was qualified, the average pass rate was 94.01%; among 111 samples in 23 internal medicine clinics, 99 was qualified, the average pass rate was 89.19%; among 110 samples in 13 clinics carrying out acupuncture or other invasive treatments, 106 was qualified, the average pass rate was 96.36%; the detection of 18 samples in 7 clinics not carrying out invasive treatments indicated that 15 was qualified and the average pass rate was 83.33%.III. The comparison between disinfection effectiveness in 2013 and 2014Total qualified rate of disinfection status in private clinics was 89.48% and 93.12%, respectively in 2013 and 2014. Qualified rate of medical staffs’ hand hygiene was 82.35% and 84.62%, respectively. There was no significant different between them(P > 0.05). The disinfection effectiveness of clinic countertops in 2014 was increased in comparison with 2013(qualified rate: 99.21% vs 92.26%, P < 0.01). Disinfection status in internal medicine clinics and clinics not carrying out invasive treatments improved obviously. Qualified rate of UV light irradiance was 83.19% and 90.32% in 2013 and 2014, respectively. The difference was statistically significant(P < 0.05). Among all clinics of different type, dental clinics were most significantly improved. In 2013 and 2014, the average pass rate for sterilization of medical devices was 98.85% and 98.99%, respectively. There was not significant different between them.Conclusions:The total disinfection status of private clinics in Gusu district of Suzhou City was acceptable, however, medical staffs’ hand hygiene and the qualified rate of ultraviolet light were not satisfied. The training of medical staffs’ hand hygiene and the routine use of ultraviolet light disinfection should be strengthened. The disinfection effectiveness in dental clinics, internal medicine clinics and other clinics carrying out invasive treatments was acceptable, while the status in private clinics not carrying out invasive treatments was unsatisfied due to ignoring disinfection and isolation measures. Medical staffs’ hand hygiene in private clinics was not significantly improved in 2014. However, the disinfection status of clinics countertops was improved, especially in internal medicine clinics. And the UV light irradiance was also significantly improved in 2014 in private clinics, especially in dental clinics. The disinfection status of private clinics was effectively enhanced by two consecutive years of monitoring and reduced the risk of iatrogenic infection.
