Cognition And Management Situation Of Cancer-related Breakthrough Pain And Its Intervention Research Among Nurses In Hengyang City

Objective To provide the reference basis for hospitals in terms of specialist education of breakthrough pain(BTP) and training nurses for the capabilities of BTP management, the current situation of perception and management of BTP among nurses were analyzed at the Oncology Department of the third-level grade-A hospitals in Hengyang city, the management training and its effects were studied. Methods1 The cognition and management situation of BTP among 160 nurses, who were from four third-grade class-A hospitals in Hengyang city of Hunan province, were investigated by the questionnaire of cognition and management of BTP for nurses. The avolid questionnaire results were analyzed by SPSS 18.0.2 On the basis of the first part of this research, the training programs for BTP management were designed. By premeasurse-postmeasure design experimental study, 40 nurses in the Oncology Department of a third-level grade-A hospitals in Hengyang city, were given four-week training(a total of 20 periods) for BTP management. Then the effects of BTP management training for the nurses were assessed. Before and after training, nurses gave pain management of 2 groups of patients with breakthrough pain, respectively. According to the pain assessment contents recorded by the nurses, their education condition to patients’ pain, the patients’ pain degree and frequency, and their satisfaction for pain management were made by nurses, the result of BTP management behaviors from nurse was evaluated after training. Results1 A total of 154 avolid questionnaire were collected. The total score of BTP knowledge was(6.77 ± 2.69), and the average correct answer was 37.6%, which was at a low level.2 There were statistically significant differences for the BTP knowledge scores of nurses among different years of working, position, hospital departments as well as experience of BTP training(P < 0.05), but not among different educational level of nurses(P > 0.05).3 After training, the total score of BTP knowledge was(15.30 ± 1.31), which was higher than that made before the training, whose difference was statistically significant(P < 0.05). After training, the assessment content of nurses for the BTP was more comprehensive and accurate; They could teach the test group patients more specifically. The rate of satisfaction level of health education of test group patients was 86.7%, which was higher than that of control group patients, the difference was significant statistically(P < 0.05). The pain degree and the satisfaction for analgesic effect of the BTP in both groups showed no significant difference(P > 0.05); The frequency of daily BTP of control groups was higher than test group(P < 0.05); The satisfaction level for BTP care between two groups was statistically and significantly(P < 0.05). Conclusions1 The cognition of BTP knowledge among nurses was at a low level at the Oncology Department of the third-level grade-A hospitals in Hengyang city. The influencing factors included years of working, position, department and BTP training experience.2 By developing training for BTP management to nurses in the Oncology Department, the BTP knowledge of nurses can be improved, and the assessment and health education skill can be enhanced, so as to improve the satisfaction of patients for BTP management.
