| Objective:To study the risk factors to left ventricular hypertrophy and the changes of its function in patients with essential hypertension associated with obstructive sleep apnea hypopnea syndrome (OSAHS).Method:499 hypertensives including 321 males and 178 females from the First Affiliated Hospital of Xinjiang Medical University were enrolled. According to the apnea hypopnea index (AHI),the patients were divided into 4 groups:AHI<5/h for the hypertension group (EH group, n=24),5~15/h for the hypertension associated with mild OSAHS group (mild group,n=208), and> 15-30/h for hypertension with moderate OSAHS group(moderate group, n=156), AHI>30/h for hypertension with severe OSAHS group (severe group, n=111). The parameters of left ventricular structure and its systolicã€diastolic function determined by echocardiogram were compared among each groups.One way analysis of variance and the multi-factor Logistic regression analysis were used to study the influencing factors of the observed variables.Results:(1) The baseline date of gender, age, BMI, smoking and alcohol drinking rate, blood pressure were significant difference between the four groups (P<0.05). (2) All the left ventricular parameters were also significant difference between the four groups(P<0.05). LVEDD, IVST, LVPWT, LVMI in the moderateã€severe group were higher than those in the mild group and simple hypertension group,and LVEDDã€IVST in these severe group were higher than the moderate group (P<0.05); E/A in the severe group was lower than that in the mild and simple group (P<0.05);LVEF in severe group was higher than that in the mild group (P<0.05).(3)The multi-factor Logistic regression analysis showed that sex (OR=0.24,95%CI,0.116-0.497), blood pressure (OR=2.256,95%CI 1.22-4.172), AHI (OR=2.115,95%CI1.074-4.165) were the influencing factors of LVMI; AHI (OR=2.212,95%CI 1.018-4.807) was the influencing factors of LVEF; age (OR=2.723,95%CI 1.98-3.744),blood pressure (OR=1.627,95%CI 1.071-2.172), AHI (OR=1.786,95%CI 1.132-2.819) were the influencing factors of E/A.Conclusion:Female, blood pressure,AHI as risk factors for left ventricular hypertrophy; AHI as a risk factor of left ventricular systolic function;age,blood pressure, AHI as risk factors of left ventricular diastolic function. |
