In the recent years, medical dispute showed an increasing trend, the doctor-patient relationship has become one of the biggest problems of the medical area. The paper use Principal-agent theory to find the reason of medical dispute and use the Extended Expectation Disconfirmation Model to analyze the expectation of patients. So that we can learn how to reduce the incidence of medical disputeThis paper considers that the agency problem is the main reason of medical dispute which is mainly due to the incompleteness of the contract. Firstly, this give the agent the advantages of using the information to deceive the principal, so that the doctor do not chose the action which could bring the patient the maximum benefit during treatment. Secondly, it may also cause the situation that the principal misunderstand the effort of agent, resulting in the patient’s excessive expectations. The purpose of this paper is to study from the perspective of both patients and physicians, reducing the incidence of medical dispute.For doctors, the article expand the traditional principal-agent model and added medical dispute liability factor in the model, so that the model is applicable to medical service behavior. By doctors and patients’s Game Behavior, we work out the medical dispute liability factor Pareto optimal solution and designed the Pareto optimal medical contract and spread risk at the same time. So that we can avoid opportunism and bounded rationality which lead to violations or incompetence behavior. For patients, the article designed the patient’s expectations and the actual perceived value to 7 one-level indicators and 32 secondary indicators, then we work out the patient’s expectation gap through empirical analysis and multi-level fuzzy comprehensive evaluation. Then we use the Extended Expectation Disco nfirmation Model to measure in which medical link has the biggest expectation gap and which factor is the main reason that lead to medical dispute. Finally, the article try to minimize the gap between the expected value of the actual service of the doctor by the management of expectation gap, so that to prevent medical complaints and disputes, and guide patient expectations to maintained at a normal level and the patients will experience medical services which is better than expectation and feel satisfact.
