Serum CyPA Level Change In Patients With Coronary Heart Disease And Its Clinical Significance

Objective: To observe the level of cyclophilin A(Cy PA) and matrix metalloproteinase-9(MMP-9), C-reactive protein(CRP) in patients with coronary heart disease(CHD), and compare their relationships, as well as analyse the serum levels of Cy PA and coronary heart disease, atherosclerotic plaque stability and the relationship between severity of coronary artery disease, and explore the clinical significance of the Cy PA level changes in patients with coronary heart diease.Methods: All subjects were enrolled form inpatients who underwent coronary angiography.We studied total of 90 patients. These included 64 patients with CHD. There were divided into there groups according to the clinical types:22 cases of stable angina(SAP groups),20 cases of unstable angina(UAP groups),22 cases of acute myocardial infarction(AMI groups);There were divided into three groups according to the coronary artery lesion count: single lesion group(23 cases),double branch lesion group(19 cases),multi vessel lesions in the group(22 cases);while 26 controls with normal coronary arteries were select as control groups. There were divided into four groups according to Gensini score: normal control group, mild coronary lesion group(<30),moderate coronary lesion group(30-60) and severe coronary group(≥60).Serum levels of cyclophilin A(Cy PA) and matrix metalloproteinase-9(MMP-9) were detected by ELISA. Immune scattering turbidimetry test C-reactive protein(CRP) concentration. Experimental data were processed by SPSS 18.0 statistical analysis software. A value of P<0.05 was considered statistically significant.Results:1.The serum level of cyclophilin A, matrix metalloproteinase-9 and C-reactive protein was significantly higher in patients with CHD than those in the control groups(P<0.01).2.The serum Cy PA,MMP-9,CRP level in AMI groups and UAP groups is significantly higher than those in control groups and SAP groups(P<0.01); The serum Cy PA, CRP level was no statistically significant difference between SAP groups and control groups(P>0.05); The serum Cy PA,MMP-9 level was no statistically significant difference between AMI groups and UAP groups(P>0.05); 3.The single lesion, double branch lesion group and multi vessel lesions of Cy PA,MMP-9,CRP levels is significantly higher than the control groups(P<0.01 or P<0.05), Cy PA,MMP-9,CRP level along with the increase in coronary artery lesion count and elevated(P<0.01 or P<0.05);4.The mild lesion group, moderate lesion group and severe lesion group of Cy PA,MMP-9,CRP level is significantly higher than the control groups(P<0.01); The moderate lesion group and severe lesion group of Cy PA,MMP-9,CRP levelis significantly higher than the mild lesion group(P<0.01);The severe lesion group of Cy PA, CRP level is significantly higher than the moderate lesion group(P<0.05).5. Cy PA,MMP-9 and CRP levels in serum of patients with CHD with Gensini score of coronary lesions were positively correlated(r=0.489,P<0.01;r=0.473,P<0.01;r=0.535, P<0.01); The serum Cy PA levels in patients with CHD with MMP-9 and CRP levels were positively correlated(r=0.772,P<0.01;r=0.749,P<0.01);Serum MMP-9 levels in patients with CHD and CRP levels were positively correlated(r=0.938,P<0.01).Condusions:1.The level of serum cyclophilin A in patients with CHD is apparently higher than those in control group,while the level in acute coronary syndrome patients increase more significantly.The change of the level cyclophilin Acould be taken as the reference index of ACS diagnosis to some extent. 2. The level of serum cyclophilin A in patients with acute coronary syndrome significantly higher than that of patients with stable angina pectoris,which suggests this may be related to instable coronaryatherosclerotic plaque. 3. The level of serum cyclophilin A is positively correlated with severity of coronary artery lesion, and more coronary artery disease, and the more serious coronary artery lesions are, the higher levels they are. 4. The level of serum cyclophilin A is positively correlated with Gensini, MMP-9 and CRP levels in patients with coronary heart disease, and it may play an important role in the inflammatory reaction of coronary artery. The serum level of Cy PA might play an important role in evaluating the condition of patients with coronary heart disease.
