Objective: This study is designed to investigate the difference of peripheral serum hs-CRP , PAPP-A, MMP-3 levels in different subtype of ACS and correlation between hs-CRP, PAPP-A and MMP-3 levels and explore the value of inflammation markers in the risk stratification of ACS in clinical work.Method: 70 patients(choose from 301 hospital, period from October of 2003 to March of 2004) with CHD were divided into stable angina(SA) group(12 patients),unstable angina (UA) group (20 patients),acute myocardial infarction (AMI) group (38 patients), after check the clinical presentation, electrocardiogram, biochemical markers for myocardium necrosis.20 subjects (choose from 301 hospital )who underwent body check but showed no evidence of CHD served as normal control group. The levels of serum hs-CRP were measured by immunoturbidimetry assay and MMP-3 , PAPP-A were measured by ELISA.Results: serum hs-CRP was increased stepwise according to the sequences of control group, SA group , UA group , AMI group. Compared with the control group, serum hs-CRP level was markedly increased in ACS patients ( P<0.05, P<0.05), and no significant difference was found between AMI group and UA group, and no significant difference was found between SA group and the control group (P =0.768) . serum PAPP-A was increased stepwise according to the sequences of control group, SA group , UA group , AMI group. Compared with the controlgroup, serum PAPP-A level was markedly increased in ACS patients ( P<0.0\, P<0.05), and significant difference was found between AM] group and UA group( P=0.02), and no significant difference was found between SA group and control group (P =0.418) .serum MMP-3 was increased stepwise according to the sequences of control group, SA group^ UA group -, AMI group. Compared with the control group, serum MMP-3 level was markedly increased in ACS patients ( P<0.01. F<0.01), and no significant difference was found between AMI group and UA group, and no significant difference was found between SA group and the control group (P =0.698). The correlation analysis in ACS group showed that there was no correlation between hs-CRP and MMP-3 (r =0.099, P=0.539) . There was no significant correlation between hs-CRP and PAPP-A (r =0.017, P=0.210) . There was a significant correlation between MMP-3 and PAPP-A (r =0.250, Z^O.046) .Conclusions: This study showed that serum hs-CRP -. PAPP-An MMP-3 levels were elevated in ACS group, suggesting the serum hs-CRP % PAPP-A > MMP-3 levels may reflect the severity of ACS and inflammation factors have relationship with plaque unstable and reflect the activities of plaque indirectly.
