ObjectiveThe purpose of this study is to observe the effect of the xingnaojing injection on the hot-closed comatose patients according to the TCM differentiation. This paper discusses the effect of this kind of medicine through comparing the changes of glasgow coma scale (GCS) and bispectral index (BIS), and to evaluate the application of BIS in the conscious state monitoring of the patients with disturbance of consciousness through the relevant research of GCS and BIS.MethodsThe paper includes 48 cases of hot-closed comatose patients according to the TCM differentiation as the research object, and they are randomly divided into two groups--the experimental group and the contrast group both of which are monitored with BIS. Both of the two groups received the conventional western medicine treatment (including opening airway, stabilizing vital signs, etiological treating, improving cerebral circulation, controlling brain edema, reducing the intracranial pressure, anti-infection, anti-convulsion, nourishing brain cells, the basic nutritional support, nursing support and other symptomatic treatment and so on). In addition to the conventional western medicine treatment, the treatment of experimental group was combined with the xingnaojing injection (20ml qd). After that, the author observed the changes of BIS and GCS of two groups of patients at the same time before and after using the drugs (starting point of time is defined as the initial use of the medication), and the observation lasted for 7 days. And then we collect data and analyse them by statistics.Results1.Through the contrast of the baseline information of two groups of patients (including gender, age, APACHE Ⅱ、GCS and the time of onset to treatment before the treatment), the difference has no statistical significance (P>0.05), it means two groups of data are comparable.2. The contrast in the groups before and after the treatment shows that GCS and BIS of both groups of patients have risen to some extent, it means that the difference has statistical significance (P<0.05)3. The contrast between two groups after the treatment shows that GCS and BIS of the experimental group are all higher than that of the contrast group, it means that the difference has statistical significance (P<0.05)4. On the 7th day, (when the observation period ended), the conditions of patients from the experimental group were better than that of the contrast group,the difference has statistical significance (P<0.05)5. Before the treatment, all patients with BIS and GCS has significant correlation (r=0.949,P<0.01).After treatment, the BIS and GCS in the test group has significant correlations (r=0.851, P<0.01), and for the contrast group, the BIS and GCS has significant correlations (r=0.928, P<0.01)Conelusions1. Both conventional western medicine treatment and conventional western medicine treatment combined with xingnaojing injection can improve the condition of comatose patients.2. Compared to the conventional western medicine treatment, the combined use of xingnaojing injection could improve the conditions of comatose patients better.3. In the aspect of conscious state monitoring of comatose patients, there is positive correlation between BIS and GCS, which is worth clinical promoting.
