Objective: To observe the difference in clinical efficacy between the standardized western medicine based standardized western medicine for acute ischemic stroke without coma and the standardized western medicine based on guidelines for the treatment of acute ischemic stroke patients without coma,and further discuss the differences between the two groups.The possible mechanism of this discrepancy is discussed,so as to provide evidence for expanding the scope of application of Xingnaojing Injection in the treatment of ischemic stroke.Methods: the selected subjects were the patients with acute ischemic stroke without coma within 24 hours from January 2017 to December 2017 in Wuhan No.1 Hospital.The number of cases was 48 cases,and they were randomly divided into the treatment group(Xingnaojing injection + standardized western medicine based on the south of the finger)and the control group.The standardized treatment of Western medicine based on guide was used to observe and record the NIHSS scores of two groups before medication,48 hours after medication,7 days after medication and 10 days after medication(National Institute of Health stroke scale in the National Institutes of Health Stroke Scale)and MRS scores before,30 days after medication and 90 days after medication(Modified)Rankin Score is an improved Rankin scale,which is usually used to assess the changes in the degree of disability or independence of stroke patients in daily activities,and to analyze the results of the two groups.Besides,it also needs to record the blood and urine routine examination of electrocardiogram and liver function before and after medication,48 hours after medication and 10 days after medication.Can(alanine aminotransferase ALT,cereal aminotransferase AST),renal function examination(blood urea nitrogen BUN,uric acid UA,creatinine Cr),blood lipid(triglyceride TG,LDL LDL,high density lipoprotein HDL),blood sugar,blood coagulation function,and the occurrence of adverse events should be recorded(adverse events).The occurrence of death,intracranial hemorrhage,recurrent cerebral apoplexy,recurrent intracranial hemorrhage,abnormal liver function and abnormal renal function were found during the period of drug use.Results: Xingnaojing treatment group could significantly reduce the NHISS score in the treatment and control group after tenth days,and the difference between the two groups was statistically significant.The difference in MRS score of the two groups was statistically significant(P<0.05)on the ninetieth day,and there was no obvious abnormal change in the changes of the blood and urine routine examination,the routine examination of blood and urine,the liver function(ALT,AST),the renal function examination(BUN,UA,Cr),the blood lipid(TG,LDL,HDL),blood glucose and coagulation function in the two groups after 48 hours and 10 days.All of the two groups were not changed.There were serious adverse events(death,intracranial hemorrhage,progressive stroke,vascular events,liver function,renal impairment,and other adverse events).Conclusion: in the practical use of this test,it is proved that in the patients with acute ischemic stroke in the absence of coma,the combination of Xingnaojing injection combined with standardized western medicine based on guidelines is better than the standardized western medicine based on guidelines to improve the recovery of the impairment of nerve function.There were differences between the two groups in reducing the NHISS score and MRS score significantly.
