ObjectiveThe purpose of this research is aimed at the basis of the taking regular medication, physical therapy, bladder function training and nursing for the patients of spinal cord injury with urinate dysfunction, the experimental group adds using the lower limbs intelligent feedback training system for treatment, to observe and to analyze if there are differences of the bladder between the two groups of patients, so that to discusses the effect of lower limbs intelligence training system on the bladder function the Spinal cord injury patients and the clinical application value.Methods Select 40 hospitalized patients who have through magnetic resonance imaging(MRI), clinical manifestation and the relevant examination diagnosis to be of spinal cord injury with bladder dysfunction. They are grouped according to the principle of randomization and the successively order of the clinic time. The 40 patients are conformity with the requirements and are willing to be selected 20 cases in the patients to conduct the experiment as the experimental group, and another 20 patients is as control group, and the two groups of patients are performed routine rehabilitation therapy, Include: ①Drug treatment: nerve nutrition drug of regular doses. The corresponding antibiotics and etc. are selected for the patients who have urinary tract infection according to the results ofurine culture and drug susceptibility test; ②Exercise therapy and physical therapy: the active and passive activities of the muscle strength weakening or lose intelligent exercise training instrument, neuromuscular electrical stimulation, pressure instrument, and electric bed standing up training. The time is twice a day, 30 minutes every time; ③Bladder function nursing: the patients and their families has learned the clipping catheter on time and establishing the reasonable water and eating plan, and making the urination consciousness training by the rehabilitation nursing personnel, and have learned to use Valsalva method, Crede method to assist urination, at the same time, the rehabilitation nursing personnel also conducts the psychological nursing and health education to the patients. On the basis of the above routine rehabilitation therapy, the experimental group adds the lower limb intelligent feedback training system for training. Two groups of patients make the bladder residual urine catheter determined respectively before this experiment was carried out and after being treated for four weeks and reaching a course of treatment, and reflecting the state of bladder function by the residual urine volume. Moreover, collect the urinary tract function basic data set table record according to the International spinal cord injury data and the micturition desire of the patients before and after treatment, Frequency of involuntary discharge of urine, bladder emptying method to make the corresponding comparison. Finally, use the software of SPSS 17.0 statistics to make the analysis and overall treatment to the above collected data, and summarize the conclusion.Result 40 patients all coordinated to complete test, no adverse reactions such as orthostatic hypotension, and the gender, age of two groups of patients had no obvious difference through the statistical analysis. The difference of the patients of the experimental group and control group before bladder residual urine volume treatment was no statisticallysignificant(P > 0.05), and which was decreased, and the difference was statistically significant(P < 0.05); In the experimental group, there was 4 cases which recovered the micturition desire, and 3 case which the bladder emptying method had changed, and 5 cases which the involuntary leakage of urine frequency had changed; and in the control group, 2 cases which recovered the micturition desire, 1 case which the bladder emptying method had changed, 2 cases which the involuntary leakage frequency had changed. According to American spinal injury association(ASIA) damage degree classification, the number of the recovering of the lower degree disease patients of the observation indexes of above various items was more than the high degree disease patients.Conclusion The routine drug therapy, exercise therapy, physical therapy, bladder function training and nursing have certain help to promote the improvement of the bladder function, and the experimental data shows that on the basis of the above treatment adding with lower limb intelligent feedback training system can further recover the bladder function. The related adverse reactions are not seen in patients in the whole process of treatment, and the patient’s tolerance and adaptability is well, which has a broad application prospect. In addition, in the patients of the recovered bladder function, the number of the lower degree disease patients of the ASIA classification degree is more than the high degree disease patients. Both have correlation, it is speculated that in ASIA damage degree classification, the rehabilitation efficacy of the lower degree disease patients may be superior to the high degree disease patients.
