Objective : To investigate the expression of Neural cell adhesion molecule(CD56+) and L1 cell adhesion molecule(CD171) in the hand-foot-mouth disease(HFMD) infants with EV71 infection from the peripheral blood serum,and thus infer the expression of NK cell in the HFMD infants with EV71 infection from the step of inherent immunity,especianlly in the central nervous system.Further,we can presume that whether the NK cell play a part in the innate immune response stage of the HFMD infants with EV71 infection,and whether it play a part in the immune escape process.We also can presume the immune expression that EV71 infect in the central nervous system.The result can provide the objective facts from the molecular immunology in the early pathophysiological process of the EV71 infection,especially in the central nervous system. The result can explain the reason that the HFMD infants with EV71 infection appear the central nervous system symptoms from the pathophysiology.It explores new ideas for the prediction of severity in the HFMD infants with EV71 infection, especially in the infection of the central nervous system.It provides new ideas and theory foundation for creating specially good effect biological preparation for curing EV71 infection.Methods : Sixty cases of the HFMD infants with EV71 infection children were choosen from age,disease time,Pre hospital treatment,the birth history,the history of feeding,growth history,past medical history,history of preventive inoculation,family history,earlier infectious disease contact history,the situation of residence,the situation of the living environment,habits of eating,work and rest time,the economic situation of family,the education level of parent who took care of child. The children who were selected aged 1 to 3 years old,and didn’t have Pre hospital treatment.The overall sex ratio was 1:1.They didn’t have special personal history,past history or family situation.They were divided into two groups(mild group and severe group),accourding to the Enterovirus 71 Infection In Severe Cases Of Clinical Treatment Expert Consensus(2011).Thirty cases of healthy children were identified as the control group with the same time collecting samples,same ages,similarity personal situation,similarity family situation,similarity family environment.All children were collected venous blood samples with limosis four to six hours,using the same method collecting blood samples.The collection tubes and the blood sampleing needles were from the same factory and batch.Children were not intense crying during the blood collection.The blood collection were successful once time.All children with the HFMD infants with EV71 infection were collected blood samples before the treament.All blood samples were accuracy from the venous,and were not hemolysis after the centrifugation.Collect 0.5ml serum in thirty minutes after the centrifugation.Refrigerate the serum under 20℃ rigorous according the instruction of CD56+,CD171’s Elisa kit.Do not do freezing and thawing operation repeatedly.Rewarm the serum with the room temperature before the testing.Test the level of CD56+,CD171 from each group serum with ELISA determination technique of double antibody sandwich method.Draw the detection curve with the standard sample from the Elisa kit.Calculate every sample’s concentration according the standard curve.Statistical study with SPSS 13.0.The measurement data was expressed with x±s.The data was compared with the rank sum test and the single factor variance.Results:The severe group CD56+(24.67±2.41ng/L)and the mild group CD56+(36.53±0.92ng/L)were reduced comparing with the control group CD56+(40.10±1.31ng/L),and there was statistical significance with each group. The severe group CD171(16.53±1.70ng/L)and the mild group CD171(24.05±0.69ng/L) were reduced comparing with the control group CD171(27.25±0.71ng/L),and there was statistical significance with each group.Conclusion:The NKcell from the HFMD infants with EV71 infection children had the abnormal situation. The number of the NKcell from the HFMD infants with EV71 infection children was discounted with the healthy group.The activity of NKcell from the HFMD infants with EV71 infection children was reduced with the healthy group.The transformation was positive correlation with the severity of the disease. The number of the NKcell in the central nervous system from the HFMD infants with EV71 infection children was discounted with the healthy group.The activity of NKcell in the central nervous system from the HFMD infants with EV71 infection children was reduced with the healthy group. The transformation was positive correlation with the severity of the central nervous system symptoms.The results expressed that the number and the activity were reduced in the stage of innate immunity of he HFMD infants with EV71 infection.The abatement affected the NKcell’s immune response including antibody dependent cell cytotoxicity,natural cytotoxicity and the ability of producing cell factor.As a result,the organism’s immune defense ability was reduced against the EV71 infection. The abatement of the number and the activity of the NKcell expressed a supplementary role in the immune escape process after the EV71 infection,especially in the infection of the central nervous system.The NKcell’s activity and the ability of composing and producing the cell factor decreased by the molecular immunology.It provided evidence about the NKcell after the EV71 earlier infection to prove that the level of kinds of cell factors were fluctuated, especially for the pathophysiological changes in the central nervous system by the molecular immunology.Based on the cell factor,It showed the reason that the child with EV71 infection appeared the central nervous system symptoms by the pathophysiology.It provided a small sample data living example which can calculate the systemic immune response,the severity of the HFMD infants with EV71 infection,especially the severity and prognosis of infection of the central nervous system.It explored a new idea and provided a theoretical basis for further research about large data sample.It provided anew idea and a theoretical basis about earlier timely effective treatment with the HFMD infants with EV71 infection by using biological preparation for raising and affecting NKcell in greater range to strengthen innate immunity in the future.
