Clinical Observation Of Buyanghuanwu Decoction Rinsing With VSD In Chronic Limbs Wound

Objectiver:To evaluate the effect and find the advantages and the disadvantages of vacuum sealing drainage with Buyanghuanwu decoction rinsing and to provide a more effective method in dealing with chronic limbs wound, we perform this clinical observation using multiple indicators to compare vacuum sealing drainage(VSD) with buyanghuanwu decoction rinsing and VSD with physiological saline rinsing in dealing with chronic wound in the limbs.Methods:From December 2013 to December 2014,43 patients in total with chronic wound in the limbs was treated and followed up in orthopeadics department and burn department. They were randomly divided into a treatment group(23 cases) and the control group(20 cases), 23 cases treated with VSD therapy and buyanghuanwu decoction rinsing as the treatment group, and others treated with VSD with physiological saline rinsing as the control group. Record blood infection indexes, the granulation tissue growth rate, different kinds of complications encountered, the time needed before skin-grafting, the survival rate of first time skin-grafting, the time of treatment, compare the records by statistics methods and draw the conclusion.Results:1. The difference of general information between the treatment group and the control group was no statistical significant (P>0.05).2. The differences of WBC,ESR had no statistical difference in admission day and 3,7 day postoperatively (P>0.05), while 14 day postoperatively the difference of WBC,ESR was statistically significant (P<0.05);the difference of CRP between the groups had no statistical significance in admission day and 3 day postoperatively (P>0.05), in 7,14 day postoperatively the differences of CRP between the groups was statistical significant (P<0.05).3. The difference of the granulation tissue growth rate 1 week after the debridement surgery was statistical significant(P<0.05), whie the rate was 83.89%±3.70% in the treatment group compared with 72.49%±2.03% in the control group.4. The treatment group bring about fewer complications than the control group. While the difference has no statistical significance(P>0.05).5.19 patients of the total 23 patients in the treatment group didn’t need to do the skin-grafting operation again; and the number is 11 of 20 in the control group, statistic analysis shows the difference was statistically significant (P<0.05).6. The time from the debridement to skin-grafting operation in the treatment group is 13.0±3.55 days, compared with 19.1±3.19 days in the control group, the difference had statistical significance(P<0.05). whilie the total time in hospital is 29.9±6.4 days, compared with 39.6±11.8 days in the control group. the difference was statistically significant (P<0.05).Conclusion:1. Buyanghuanwu decoction rinsing with VSD has advantages when infection indexes lower,the granulation tissue growth faster, time shorter, growth better compared with VSD with physiological saline rinsing,which greatly improves the survival rate of the graft and thereby improve the clinical efficacy,and dosen’t increase the occurrence of the complications.2. Buyanghuanwu decoction rinsing with VSD for the treatment of chronic infectious limbs wound overall efficacy satisfaction.
