Objective:To study the clinical effect of manipulative reduction combined with percutaneous tension band for the treatment of patella fracture. To find a treatment of patella fracture that less invasive, fewer complications, simple manipulation, effective and reliable, and it is worth of clinical application.Methods:From March 2013 to June 2014, A total of 60 patients with patella fracture were chosed from The Wendeng Osteopath Hospital of Shandong Province, All patients were randomly divided into two groups, group A (treatment group) and group B (control group), 30 cases in each group. Group A were treated with manipulative reduction combined with percutaneous tension band, and group B were treated with open reduction combined with tension band. After which both group had a plaster or brace fixation. And use antibiotics 1-3d. A week later, removal the external fixator and exercise the knee joint. After 6-8 weeks, removal the internal fixator according to the exercise in knee function and fracture healing. After operation, all patients were regularly followed up. And evaluate the clinical effect by reference to Bostman clinical score, The data from the two groups were statistically analyzed. Such as operative time, length of stay, fracture healing time and the knee range of motion.Results:All the patients were followed up for 3~6 months, with an average of 5 months. During the follow-up period, control group 1 case with postoperative incision infection, and the wound healing after treatment, the rest does not appear that limb pinhole or incision infection, the nonunion of fracture, necrosis of skin condition, and the affected area re-injured. On two groups of patients with operative time, length of stay, fracture healing time and the knee range of motion for statistical analysis found that in addition to the operation time is, the rest of the index in treatment group was better than control group (P< 0.05), by reference to Bostman clinical criteria for 12 weeks the knee joint function after surgery for statistical analysis found no statistical difference (P>0.05).Conclusion:From the statistics, Although the operative time and t knee joint function in 12 weeks between the treatment group and control group are quite. but the former has less invasive, less chance of infection, shorter hospitalization time, fast healing, need not take out a fixed cut again widely, and no obvious postoperative scar, is an ideal treatment method, is worth popularizing in clinical application.
