Background:For Osteoporotic Vertebral Compressive Fracture conservative treatment are hard to suffer for patients with infaust curative effect and major complications. At the mean time, discission and internal fixation are limited by osteoporosis with difficulties coming one after another. The key to the treatment should be to relieve pain quickly and do functional exercises at the early period to prevent the complication of long-term bed rest. So the minimally invasive therapy for Osteoporotic Vertebral Compressive Fracture coming,such as vertebroplasty and kyphoplasty,they can resolve this single treatment's disadvantage properly. However, PVP can not be very effective in vertebral body height restoration and kyphosis correction. PKP can overcome the disadvantages of PVP, but its cost is high.Objective:To explore the effect of manipulative reduction in Percutaneous Vertebro-plasty(PVP) for treatment of osteoporosis vertebral compression wedge shape fractures.Methods:40 patients with osteoporosis vertebral compression wedge shape fractures were divided into two groups.one group were treated with manipulative reduction and then vertebroplasty was perfomed on the fractured vertebra; the other group were not treated with manipulative redution and kyphoplasty was perfomed directly. The results of pre-operation and post-operation were quantitatively evaluated arcording to the concept of estimated vertebral height loss and vertebrotic angle of Vertebral Compression fractures.Results:Operations on the 40 vertebraes are successful..Bone cement leakage happened in 7 cases without complications. Postoperative pain scores, vertebral body heights and Cobb angle improved significantly after the surgery (P<0.05), but the change of Cobb angle and VAS score after operation between the two groups was no statistically significant(P>0.05).Conclusion:The manipulative reduction with vertebroplasty was effective in the treatment of osteopnmsis vertebral compression wedge shape fractures and restore the anterior borde of compressed vertebral body especially. Manipulative reduction got good effect on vertebral compression fracture. We can draw a conclusion that manipulative reduction with vertebroplasty have favourable effect on Osteoporotic Vertebral Compressive Fracture treatment.
