Objective: To observe the changes of serum HMGB1 and hs-CRP in T2 DM complicated with lacunar cerebral infarction patients,and to investigate the relationship between HMGB1 and hs-CRP levels and clinical features.Methods: The fasting serum HMGB1 and hs-CRP levels were measured in 60 T2 DM patients(1999 WHO diagnostic standards), including 38 diabetic patients without lacunar cerebral infarction, 41 diabetic patients with lacunar cerebral infarction.35 healthy individuals(control group). Investigate all the research object of lifestyle and medical history, Meanwhile we measured the weight, height, waist circumference(WC) Systolic blood pressure(SBP) diastolic blood pressure(DBP). Fasting blood glucose,plasma lipid(including TC,TG,LDL-C,HDL-C), Hb A1 C, fasting insulin were also measured. Calculate all the research object of body mass index(BMI) and related index. Homeostasis model assess insulin resistance index(HOMA-IR). Serum HMGB1 and hs-CRP levels by enzyme-linked immunosorbent assay(Enzyme-linked immunosorbent assay, ELISA). Compare each group index differences, further analysis of serum HMGB1 and hs-CRP with T2 DM with lacunar infarction group each clinical relevance of indicators. Multiple stepwise regression analysis of the influence factors of serum HMGB1 and hs- CRP.Results: 1.T2 DM and T2 DM with lacunar cerebral infarction groups Compared with control group:①The levels of FBG, FINS, HOMA-IR,Hb A1 C, TC,TG,LDL-C in T2 DM with lacunar cerebral infarction were higher than in control group, the levels of HDL-C in T2 DM with lacunar cerebral infarction were lower than in control group( P<0.05).②The levels of FBG, FINS,HOMA-IR,Hb A1 C,TC,TG,LDL-C in T2 DM without lacunar cerebral infarction group were higher than in control group, the levels of HDL-C in T2 DM without lacunar cerebral infarction were lower than in control group( P<0.05).2.T2 DM group compared with T2 DM with lacunar cerebral infarction group:The levels of FBG, FINS, HOMA-IR,Hb A1 C,TC,TG,LDL-C in T2 DM with lacunar cerebral infarction were higher than in T2 DM without lacunar cerebral infarction group( P<0.05).3. The levels of HMGB1 and hs-CRP in each groups: The levels of HMGB1 and hs-CRP in each groups was T2 DM with lacunar cerebral infarction group>T2DM without lacunar cerebral infarction group>control group( P<0.05).4.Correlation analysis: FBG, FINS, HOMA-IR, Hb A1 C,TG,TC,LDL-C had significant positive correlation with Serum HMGB1 and hs-CRP(P<0.05), and HDL-C had negative correlation with HMGB1 and hs-CRP(P<0.05).HMGB1 and hs-CRP had a strong positive correlation(P<0.01).5. Multiple regression analysis: FINS,HOMA-IR,TG,TC,LDL-C were the risk factors of HMGB1(P<0.05); HOMA-IR,TG,TC,HDL-C were the risk factors of hs-CRP(P<0.05).Conclusion: 1. The serum HMGB1, hs-CRP levels in T2 DM without lacunar cerebral infarction were higher than that in control group,and lower than that in T2 DM with lacunar cerebral infarction, it suggested that HMGB1 and hs-CRP may had certain role in the occurrence and deve lopment of T2 DM and T2 DM with lacunar cerebral infarction.2.TC,TG,LDL-C,HOMA-IRhad significant positive correlation with Serum HMGB1 and hs-CRP, and HDL-C had negative correlation with HMGB1 and hs-CRP.HOMA-IR,TC,TG are significant determinants of serum HMGB1,hs-CRP levels. Speculated that HMGB1, hs-CRP may play the biological role by influencing insulin sensitivity and lipid metabolism.3. Serum HMGB1 and hs-CRP levels were strongly positively correlated, indicating that between HMGB1 and hs-CRP exist on its mechanism of action synergy.
