Objective:Periodontitis is a chronic inflammatory disease that is mediated by host-parasite interactions.Chronic periodontitis is initiated and sustained primarily by bacterial plaque.A common clinical characteristic of chronic periodontitis is the correlation between the amount of destruction and the presence of local factors such as subgingival calculus and plaque.The purpose of this study was to evaluate the clinical and inflammatory changes as well as the reaction after treatment in patients with periodontitis when comparing traditional subgingival scaling with subgingival scaling using perioscopy.Methods:12 patients with moderate or severe chronic periodontitis were selected and divided into group A (treated with perioscopy aided subgingival scaling) and group B (treated with traditional subgingival scaling). Another 5 healthy people were selected as group C.The occurrence of gingival swelling was recorded during 1-3 day after treatment,while the pain of gingiva was evaluated by visual analogue scale(VAS) at the same time.Clinical parameters including PD and BI were recorded before treatment and 3 months after treatment.Gingival crevicular fluid of group A and group B were collected before treatment and 1,2,3 months after the treatment completed.The levels of inflammatory mediators including IL-1β, IL-17, TNF-a, PGE2 and hsCRP were analyzed by ELISA.Results:The occurrence of gingival swelling and VAS of group A and group B both decreased in 3 days after treatment,and the VAS in group A were lower than group A(P<0.05),but the occurrence of gingival swelling in these two groups showed no statistically difference.The levels of IL-1β, IL-17, TNF-a, PGE2 and hsCRP in GCF after 1,2,3 month both decreased compared with the levels before treatment,and group A decreased more than group B,which has a statistically difference. The level of inflammatory mediators in group A 3 months after treatment showed non-significant difference compared with group C. PD,BI showed significant reduction 3 months after treatment,while AL showed no significant improvement. There was no significant difference of clinical parameters between group A and group B.Conclusion:Both perioscopy aided subgingival scaling and traditional sudgingival can improve the inflammatory of periodontitis,while treatment with perioscopy can reach better outcomes.
