OBJECTIVET:To inquire into the association between the diagnosis of colonoscopy in the patients with colon precancerous lesions and the tongue manifestation. Combining with traditional Chinese medicine, tongue manifestation, some new clues may be found out about colon precancerous lesion, so that can help us to improve the screening rate of colon precancerous lesion.METHOD:To photograph the tongue of the patients that the fist time go thought colonoscopy, under nature light or white light, in a shooting angle that slightly upper than patients’tongues, in the distant of 14cm from the tongue, in the auto mode, with the Canon power shot.G1X mark II digital camera by a certain person, during the observation period (from June,2014 to December, 2014) in Dongguan Traditional Chinese Medical hospital. Tongue manifestation should be diagnosed, according to the 6th edition of "Diagnostics of Chinese Medicine", by two attending doctors that reached an agreement for the conclusion, at the same time. Then a certain person put the conclusion of tongue manifestation down with the names of patients. The observation of tongue should be completed before patients take laxative. To reference the 2nd edition of "Practical Colonoscopy" as colonoscopy diagnostic criteria. According to the conclusion of colonoscopy, pathologic diagnosis and age distribution,80 cases of normal colon mucosa as control group 1; 40 cases which were diagnosed with colorectal cancer by colonoscopy and confirmed by pathologic as colorectal cancer group; 80 cases which were diagnosed with colon precancerous lesions by colonoscopy and confirmed by pathologic as test group. To use Chi-square test for statistical analysis, investigate the correlation between them.RESULTS:1. Comparing with test group, P<0.01, the tongue color of colorectal cancer group was statistically significant. Pink tongue is common in test group. Purple tongue is more common in patients that were diagnosed with colorectal cancer.2. Comparing the tongue coating classification with control group, P<0.01, patients with colon precancerous lesions is more comment with coated tongue.3. Comparing the tongue coating classification with colorectal cancer group, P=0.003, the test group was significantly different. Greasy coating tongue is more common in patients that were diagnosed with colorectal cancer.4. Comparing the thickness of the tongue coating with control group, P<0.001, the coating thickness on the tongue of test group was significantly different Thick coating tongue is more common in test group.5. Comparing the thickness of the tongue coating with test group, P<0.001, the coating thickness on the tongue of colorectal cancer group was significantly different. Thick coating tongue is more common in colorectal cancer group.6. Comparing the color of the tongue coating with control group, P=0.027, the coating color on the tongue of test group was significantly different. Tongue with color coating is more common in test group.7. Comparing the color of the tongue coating with test group, P=0.002, the coating color on the tongue of colorectal cancer group was significantly different. Tongue with color coating is more common in colorectal cancer group.8. Comparing the distribution of the purple tongue and other color tongue among 3 group, P<0.01, the difference has statistical significance.64% purple tongue was from colorectal cancer group, which indicated that patients who with purple tongue is likely to has colorectal cancer group.9. Comparing the distribution of the purple tongue and color thick coating tongue among 3 group, P<0.01, the difference has statistical significance. 89% color thick coating tongue was from test group and colorectal cancer group, which indicated that patients who with color thick coating tongue may probably have colorectal cancer or colon precancerous lesions. Purple tongue is common in colorectal cancer group, account for 55%, which indicated that patients who had purple tongue is probably have colorectal cancer.10. Comparing the distribution of the purple tongue and purple tongue with color thick coating among 3 group, P=0.049, the difference has statistical significance. Purple tongue with color thick coating tongue is more common than purple tongue in colorectal cancer group, which indicated that patients who had purple tongue with color thick coating is likely to have colorectal cancer.CONCLUSIONS:The diagnosis of colonoscopy in the patients with colon precancerous lesions and the tongue manifestation has some kind of correlations. Comparing with the tongue color, patients with colon precancerous lesions had no significant difference with those with normal colon mucosa, however, those who with coated tongue is likely to have colon precancerous lesion. With color thick coating on the tongue is more likely to have colon precancerous lesion. Patients who had purple tongue with color thick coating is likely to have colorectal cancer. The change of suggesting that the tongue manifestation may be a prediction of colonoscopy diagnosis of colon precancerous lesions in certain degree and it can help us to improve the screening rate of colon precancerous lesions in order to obtain a perferable way to reduce colorectal cancer.
