Observation Of The Therapeutic Effects Of Ginseng, Poria And Bighead Atractylodes Powder For Macular Edema After Phacoemulsification Surgery

Objective To observate the difference of the therapeutic effects of taking the decoction of a TCM prescription names "Ginseng, Poria and Bighead Atractylodes Powder" orally besides using the eyedrops of Tobramycin and Dexamethasone (Tobra Dex(?)) between those of using the eyedrops of Tobra Dex(?) for Phacoemulsification(PHACO) and IOL surgery.MethodsUsing a prospective and experimental science study method to collect 240 patients of having had PHACO and IOL surgery who meet the inclusion criteria. All the cases are chose from the patients who take the ophthalmologic opration during Jun 2014 to Oct 2014 in Zhongshan Hospital of TCM. We devided the 240 cases into a research group and a control group and distributed 120 patients to each of them. Both of the groups have been included 4 disease category:30 cases of pute senile cataract,30 cases of cataract combined with background DR,30 cases of cataract combined with acute angle-closure glaucoma and 30 cases of cataract combined with pathologic myopia. Patients of the research group drank the decoction voluntarilly besides using the eyedrops of Tobra Dex(?) after the surgery, and those of the control group used the eyedrops merely. We use the data of vision acuity and the thickness of the macular retinal neurological cortex measured by OCT prior to the operation and those of 1 week and 1 month after the operation to observe if the difference is statistically significant.Results1. The difference of vision acuity variation of 1 week after the operation between the research group and control group of the 4 disease categories is not statistically significant(P>0.05). The difference of vision acuity variation of 1 month after the operation between the two groups of cases of senile cataract and cataract combined with pathologic myopia is not statistically significant (P>0.05). The difference of vision acuity variation of 1 month after the operation between the two groups of cases of cataract combined with background DR and cataract combined with acute angle-closure glaucoma is statistically significant(P<0.05). The improvement of vision acuity of research group is better than those of the control group.2. In 1 week after the operation, the difference of variation of the thickness of the macular retinal neurological cortex between the two groups of the 4 disease category is not statistically significant(P>0.05). In 1 month after the operation, the difference of variation of the thickness of the macular retinal neurological cortex between the research group and the control group of cases of senile cataract and cataract combined with pathologic myopia is not statistically significant (P>0.05). The difference of variation of the thickness of the macular retinal neurological cortex between the research group and the control group of patients of cataract combined with background DR and cataract combined with acute angle-closure glaucoma is statistically significant (P<0.05). The mean change of the retinal thickness of research group is less than those of the control group.3. The difference of prevalence rate of macular edema between the research group and control group of cases of senile cataract, cataract combined with acute angle-closure glaucoma and cataract combined with pathologic myopia is not statistically significant (P>0.05). The difference of prevalence rate of macular edema between the two group of cases of cataract combined with background DR is statistically significant(P<0.05). The prevalence rate of macular edema of the research group is lower than those of the control group.Conelusions1. The vision acuity of 1 month after the operation could be effectively improved by the Ginseng, Poria and Bighead Atractylodes Powder plus Tobra Dex.2. The degree of thickening of the retina 1 month after the operation could be effectively reduced by the TCM decoction.3. The prevalence rate of macular edema of patients of cataract combined with background DR could be effectively reduced by tht TCM decoction.
