Objectives To determine the characteristics of sexual behaviors and sexual partners among young female migrants working in entertainment venues, to examine the factors associated with risk sexual behavior and commercial sexual behavior among female migrants.Methods The cross-sectional survey was conducted in Luyang district and Baohe district in Hefei. Eligible criteria were female, aged 18 to 29 years, had non-Hukou of Hefei and working in sampling entertainment venues. After confirming their eligibility by screen form and getting their written informed consent and, participants were asked to anonymously complete questionnaire which were administered by face-to-face survey in a private room. The questionnaire of the survey included social demographic characteristics, the initiate sex, the sexual behavior and sexual partners during the past 6 months, the condom use with sex during the past 6 months, the utilization of HIV counseling and testing services, alcohol drinking and drug abuse.Epi Data 3.1 software and SPSS 13.0 were used to establish a data-base and analyze data. The statistical analysis technique included frequency distribution, Chi-Square test, two independent sample t test and Logistic Regression Analysis.Result A total of 358 participants completed the survey. 64.2%(230/358) participants working in affluent entertainment venues and 35.8%(128/358) participants working in average or marginal entertainment venues. The participants with an average age of 23.0 years(SD =3.0). More than half of the participants(55.6%) were got education of junior high school or less; The majority(89.7%) grown up from a nuclear family; 44.7% participants were currently married or cohabitation with boyfriend, and 53.6% reported their migrant working time was more than 3 years.Overall, 97.5%(349/358) of the sample had sexual behaviors in lifetime. Among them, 15.8%(55/349) had the first sex before 18 years old, 91.1%(318/349) had the first sex voluntary, but 81.1%(283/349) didn’t use a condom during the first sex. During the past 6 months, 89.1%(319/358) participants had sexual behavior with male partners. The percentage of participants who had sex with husband, boyfriend, commercial partners, and casual partners were 21.5%, 61.5%, 43.0% and 10.6%, respectively. 43.9% had sex with primary partners(husband and boyfriend) and non-primary partners(commercial partners and casual partners) concurrently. Among all participants, 61.2%(219/358) reported they had risky sexual behavior during the past 6 months. The proportion of condom use with commercial partners consistently is the highest(84.6%), and the condom use with husband and boyfriend were 22.0% and 3.8% accordingly. In addition, 12.8% reported ever had HIV consultation and 22.9% ever had HIV testing. Among those who had received HIV testing, 72.0%(59/82) reported they known the testing result.Among all participants, 56.7% had alcohol ahuse, 26.5% had alcohol dependence and 7.5% reported drug abuse. Participants working in affluent entertainment venues weremore likely to have alcohol abuse and alcohol dependence(68.3% and 34.3%) than those working in average and marginal venues(35.9% and 12.5% accordingly).Logistic Regression Analysis demonstrated that the risk factors associated risky sexual behavior of participants during the past 6 months including the first sex before 18 years(VS. 18 or over, OR=4.11, 95%CI: 1.45~11.61), no condom use during the first sex(VS. use, OR=4.75, 95%CI: 2.32~9.73), ever had abortion(VS. not, OR=1.83, 95%CI: 0.97~3.45). Compared with participants who only had sex with non-primary partners, those who only had sex with primary partners(OR=12.10, 95%CI: 3.63~40.34) and the both(OR=10.43, 95%CI: 3.25~33.51) were more likely to have risk sexual behavior. Participants who were currently married or living with boyfriend were more likely to engage in risk behaviors than those participants who were single, divorced or widowed(OR=2.01, 95%CI: 1.07~3.78); Participants who had more knowledge of condom use and higher condom use self-efficacy had lower risk to have risk sexual behavior(OR=0.77, 95%CI: 0.65~0.92, OR=0.82, 95%CI: 0.76~0.89 respectively).The results from multivariate Logistic Regression revealed that the influence factors of risky sexual behavior during past 6 months were different between primary partners and non-primary partners. The risk factors of high risk behavior with primary partners associated no condom use during the first sex(OR=2.17, 95%CI: 1.17~4.04) and ever had abortion(OR=1.58, 95%CI: 0.95~2.62). The risk factors of high risk behavior with non-primary partners were having 2 or more types of partners(OR=14.58, 95%CI: 4.83~43.99), had depression symptoms(OR=3.17, 95%CI: 1.12~8.95), and had alcohol dependence behavior(OR=5.26, 95%CI: 2.07~13.40). The higher condom use self-efficacy as protective factor both decrease the risk of high risk behavior of primary partners and non-primary partners(OR=0.87, 95%CI:0.82~0.93; OR=0.90, 95%CI:0.81~1.00).The multivariate Logistic Regression revealed that the risk factors of commercial sex during the past 6 months among participants including had monthly income more than 4000 Yuan(OR=3.23, 95%CI: 1.97~5.28), were diagnosed with STI or genitourinary tract infection(OR=1.76, 95%CI: 1.05~2.96) in the past year, had the first sex before 18 years(OR=1.88, 95%CI: 0.95~3.73) and had the first sex forcedly(OR=3.33, 95%CI: 1.34~8.26).Conclusions Young female migrants working in entertainment venues in Hefei had high prevalence of pre- and extra-marital of sexual behavior, and the multiple sexual partners was common among this population as well as the rate of HIV testing and counseling services utilization is low. In addition, the alcohol abuse and drug abuse have interaction with high risk behaviors, which let female migrants working in entertainments venues facing serious reproductive health threat.
