Heart Failure Caused By Secondary Hypothyroidism A Case Report And Literature Review

Objective:To study the clinical characteristics and treatment of the heart failure caused by secondary hypothyroidism.Methods:A case report of a patient who suffered from heart failure caused by secondary hypothyroidism.Design:A case report.Patient:The patient is hospitalized for heart failure. The echocardiogram showed myocardial hypertrophy, the blood test showed abnormal thyroid function and rhythm disorders of adrenocorticotropic hormone during hospitalization. After an anti-heart failure therapy the patient’s blood pressure continued to be in a low level.Intervention:The heart failure was treated with diuretic and a small dose of levothyrocine. Heart rate and blood pressure were monitored at the same time. Glucocorticoid was added after multiple examination.Result:Four month later the patient’s chest tightness, shortness of breath improved markedly. Edema of lower limbs was vanished. Renal and cardiac function was improved. Heart shadow was reduced (X-ray). Interventricular septum and left ventricular free wall was thinner than before (UCG).Conclusion:Similarly to the primary hypothyroidism, secondary hypothyroidism can also cause a series change of the cardiac structure and function. It is also can be reversed after hormone replacement therapy. Patients with secondary hypothyroidism may also have a variety of tropic hormones disorders, of which adrenal hormone deficiency is common Once levothyrocine was prescribed to treat secondary hypothyroidism, it is necessary to monitor whether there are other hormones disorders. It is very important to add corticosteroids in time. TSH levels did not change significantly in secondary hypothyroidism, and they may be decreased, normal or slightly elevated. So it is likely to be ignored and misdiagnosis.
