| Objective: Through the observation of clinical curative effect in treating PBP by acupuncture combined with skin-needle tapping in both sides of the midline neck, to find a significant therapy method which had curative effect for this disease. Besides, it could also be affordable and practical, normative and innovative, easy to popularize, then, summarize a regular pattern of treatment.Methods: After a long period of clinical practice, our team found it effective to treat PBP by acupuncture combined with skin-needle tapping in both sides of the midline neck. Adopting a randomized and contrasted method, the study chose 80 patients, who suffered from the PBP(syndrome of wind-phlegm blocking collaterals) and were all hospitalized in Acupuncture-Moxibustion Department of Traditional Chinese Medicine Hospital of Hebei Province from 2013.3 to 2014.9 The patients were divided into controlled group and treatment group with the method of random number table, and each group included 40. Control groups were treated by conventional drug(Danhong injection, Naoganjitai injection and Aspirin) treatment of stroke in Acupuncture-Moxibustion Department, and according to the condition of patients to adjustment drug, and were regulated in blood pressure, cholesterol and blood glucose during the entire process. Besides conventional drug treatment, the cured group was also treated by the acupuncture combined with skin-needle tapping in both sides of the midline neck. Acupunctured the Fengchi(both), Lianquan, Fengfu, Yamen, Tiantu, Jinjin, Yuye, Yinlingquan(both), Fenglong(both), both sides of the midline neck(experienced position, siding 1 inch from the midline of neck,from the neck starting to the clavicle fossa). Among them, both sides of the midline neck were skin-needle tapped, Lianquan was Hegu-Needled, Fengfu, Yamen, Jinjin and Yuye were fast needle and the other acupoints were needled ordinarily massaged. Counseled the acupoints before acupuncture, urged patients to sit down and make a sound and swallow when acupuncturing the Fengfu and Yamen, after withdrawal of needles, instructing patients supine, acupuncture other points, and try to take food after acupuncture. Among the above-mentioned treatment, both sides of the midline neck were needled once every 2 days, and the others were needled once every day. Retained the needle 30 minutes each time and manipulated needle once by hand every 10 minutes, then lasted 1 minute. The two groups took medication by intravenous drip for 14 days, then continued taking oral drug after treatment. When acupuncturing and skin-needle tapping in both sides of the midline neck, each course of treatment contained 10 days and the interval between courses was 1 day. After cured group’s 4 courses(43 days), observed and evaluated the effect between two groups uniformly. In order to evaluate the swallowing function, articulation function, mean velocity(VM) in cerebral artery(Vertebral artery(VA), Basilar artery(BA), Anterior cerebral artery(ACA), Middle cerebral artery(MCA), Posterior cerebral artery(PCA)) and pulsatility index(PI), patients in two groups took swallowing tests, improved Frenchay dysarthria evaluations and TCD separately before and after treatment. The obtained data were analysed by the statistical software named SPSS16.0. Among them, qualitative data were checked up by chi-square test and quantitative data were checked up by nonparametric test. If ordinal data conformed to normal distribution and had homogeneity of variance, then adopted the t-test. If ordinal data conformed to normal distribution but had no homogeneity of variance, then adopted the separate variance estimation t′-test. If ordinal data didn’t conform to normal distribution, then adopted the nonparametric test.Results: Through the statistical analysis on the sexy, age, course of a disease, the grade of Water swallow test and the grade of being improved Frenchay obstacle of voice form of the patient before treatment, we found that every index was on the level of P>0.05, had no obvious difference, and could be compared. After treatment, both patient’s grade of the Water swallow test and the improved Frenchay obstacle of voice formed were improved, and the treatment group is better than the controlled group. Through Statistical analysis, difference in both groups(before and after treatment in groups, after treatment between two groups) had the statistical significance(P<0.05). It also showed that both methods could improve the swallowing function and articulation function, and the treatment group was better than the controlled group. After treatment, the VM detected by TCD in the 5 blood vessels were all faster in varying degrees than before, but changed little in PI. Compared in controlled group before and after treatment, VA and MCA’s differences had statistical significance(P<0.05), on the contrary the BA, ACA, PCA have no significant differences(P>0.05). 5 blood vessels’ differences in PI had no statistical significance(P<0.05). Compared in cured group before and after treatment, VA, BA and MCA’s had significant differences(P<0.05), on the contrary the ACA, PCA have no significant differences(P>0.05). PI in 5 blood vessels have no significant difference either(P>0.05). Through the comparisons in VM and PI after treatment, the difference of blood vessels’ VMs in treatment group was bigger than that in controlled group. Among them, VA, BA, MCA’s differences had statistical Significance(P<0.05), instead of the ACA, PCA have no significant differences(P>0.05). The difference of PI in 5 blood vessels had no significant difference either(P>0.05). The total effective rate of treatment group was 92.5%, which was 72.5% in the controlled group. The rate of treatment group was significantly higher than the controlled group and had statistical significance in difference(P<0.05). In the process of this study, patients in both groups had steady vital signs(blood pressure, heart rate, pulse, respiration, body temperature) and nothing abnormal happened. But Symptoms of subcutaneous hemorrhage appeared on 4 patients in treatment group, then, disappeared after handling. Treatment in both groups were safe and reliable.Conclusions: Acupuncture combined with skin-needle tapping in both sides of the midline neck, can obviously improve the PBP’s symptoms, swallowing function, articulation function and mean velocity(concluding VA, BA and MCA). But affect little in the PI of VA, BA, MCA, ACA and PCA. This therapy method for curing PBP is safe and effective, affordable, and easy to operate, popularize and apply. |
